Nature:DHA这次闯祸了!科学家发现DHA被异常降解的产物是导致糖尿病视网膜病变的又一罪魁祸首

提到DHA,想必很多经常看电视的人以及有宝宝的人都很熟悉了。毕竟铺天盖地的奶粉广告中,提的最多,标注最明显的三个字母就是DHA。好像没加DHA都不好意思说自己卖的是奶粉一样,一定要把DHA标志做的够大,够显眼。

的确,鱼类中大量存在的DHA一直被认为是一种对人体非常重要的不饱和脂肪酸,也被称为“脑黄金”,是神经细胞生长及维持的一种主要成分,也是大脑和视网膜的重要构成成分。人体大脑皮层中含量占人体总量的20%,而在眼睛视网膜中所占比例最大,约占人体总量的50%。同时,DHA对婴儿智力和视力发育也至关重要。

然而,让人不可思议的是,科学家最近发现,就是这种对正常人视力至关重要的营养物质,对于糖尿病患者来说,或也是导致失明的罪魁祸首。

近日,全球顶级期刊《自然》杂志上刊登了一项超级重磅的研究,来自德国法兰克福歌德大学分子医学中心血管信号研究所的Ingrid Fleming博士带领的研究团队,发现了糖尿病引发视网膜病变的又一关键机制。

原来,在糖尿病患者以及小鼠视网膜内,一种降解DHA的酶含量显着升高,导致DHA被大量降解。而这些DHA降解产物堆积在视网膜内,可以诱导视网膜血管内皮细胞脱落,破坏血管-视网膜屏障,引发糖尿病相关视网膜病变。而抑制视网膜内降解DHA的酶的活性,则可以预防糖尿病视网膜变性的发生。



大家都知道,糖尿病往往会伴随着许多危险的并发症,而其中就包括视网膜血管损伤引起的视力障碍,也就是所谓的糖尿病性视网膜病变,这也是导致中年人失明的主要原因。然而,目前人们对于糖尿病导致视网膜病变的具体机制,还知之甚少。

目前主流观点认为,血管内皮生长因子(VEGF)在糖尿病诱导的视网膜病变过程中发挥重要作用。一些临床以及动物模型研究表明,在糖尿病患者眼内,VEGF的含量显着升高。虽然,少量的VEGF对于维持眼部血管的完整性至关重要;但是过度的表达就会产生坏处。

作为一种血管内皮细胞特异性的生成与渗漏因子,VEGF的过度表达会导致视网膜微血管渗透性的改变,导致视网膜白细胞的聚集以及血管-视网膜屏障(blood retinal barrier,BRB)的破坏,引起视网膜的炎症、渗出、出血以及水肿。

同时,靶向VEGF的药物,也早已进入临床,例如贝伐单抗(一种人源化的靶向VEGF的单抗)。然而,即使靶向VEGF的药物在临床应用过程中产生了良好的效果,仍旧有一部分患者对于VEGF抑制剂不敏感;这意味着,或许还存在其他的机制介导糖尿病相关的视网膜病变。

Fleming博士在研究过程中偶然发现,无论在糖尿病小鼠或是患者眼内,一种代谢DHA的酶的数量以及DHA代谢产物数量显着上升;同时,这也伴随着一些糖尿病性视网膜病变的早期症状。这一现象让Fleming博士感到怀疑,难道降解DHA的酶以及DHA代谢产物参与了糖尿病性视网膜病变的过程?

通过对糖尿病小鼠模型的研究,Fleming博士发现,通过增加小鼠视网膜中降解DHA酶的活性,可以使小鼠更快的出现糖尿病性视网膜病变的早期症状,例如视网膜出血水肿以及视力障碍等等。

而通过药物或基因方式抑制降解DHA的酶的活性,则成功的预防了糖尿病小鼠出现这些糖尿病性视网膜病变的早期症状。也就是说降解DHA的酶,的确在糖尿病性视网膜病变过程中发挥了重要作用。

但是,这种酶是如何导致糖尿病相关视网膜病变的呢?

这就涉及到DHA代谢产物的作用了。Fleming博士发现,正常情况下,视网膜与周围的血管之间存在着天然屏障,这道屏障由血管内皮细胞和周细胞组成。它们之间通过一些黏附蛋白紧密结合,组成一道完整的屏障,控制正常的液体和物质交换。

然而,当DHA代谢产物大量存在时,它可以改变细胞膜的生物物理学特性,降低黏附蛋白之间的结合力,削弱细胞之间的连接,最终导致周细胞从血管内皮上脱落以及血管-视网膜之间的屏障破坏,导致大量的液体以及血细胞进入视网膜,诱导炎症以及水肿,引发糖尿病相关的视网膜病变,造成视力损伤。

也就是说,Fleming博士发现了一种全新的糖尿病相关视网膜病变的机制。在小鼠体内证实了,降解DHA的酶过表达引起的DHA代谢产物积累足以诱导血管-视网膜屏障缺陷,导致糖尿病性视网膜病变,而其中没有VEGF的介导。

虽然,Fleming博士也表示,目前尚不清楚为什么糖尿病患者体内降解DHA的酶含量会显着提高;但是视网膜内大量存在的DHA无疑促进了糖尿病患者疾病的进展。这也意味着,对于正常人来说是“脑黄金”的DHA,对于糖尿病患者来说,或是失明的罪魁祸首。
