      血清学标志物如髓过氧化物酶 (MPO) 抗中性粒细胞胞浆抗体 (ANCA) (MPO-ANCA) 已被用于筛查溃疡性结肠炎 (UC) 患者。然而,MPO-ANCA 在亚洲人中显示出有限的准确性。蛋白酶 3 ANCA (PR3-ANCA) 在日本成人 UC 诊断中的表现优于 MPO-ANCA。本研究旨在评估 PR3-ANCA 在日本儿科患者中诊断 UC 的有效性。

 

      本项研究前瞻性纳入 2016 年 11 月至 2018 年 2 月期间在 12 个日本儿科中心接受评估的 17 岁以下儿童患者,并将其分为 UC、克罗恩病 (CD)、非IBD肠道疾病组 (IC) 和健康对照组(HC) 。使用化学发光酶免疫分析试剂盒分析血清 PR3-ANCA 和 MPO-ANCA的水平。研究结果显示对367名患者(148 名 UC,中位年龄为 12 岁;120 名 CD,中位年龄为13 岁;56 名 IC,中位年龄为10.5 岁;43 名 HC,中位年龄为10 岁)的血清进行了检测。UC 患儿中的中位 PR3-ANCA 值 (1.6 U/mL) 大于 CD患儿 (0.2; P< 0.001)、IC患儿 (0.15; P< 0.001) 和 HC患儿 (0.1; P< 0.001)。在接受者操作特征曲线分析中,PR3-ANCA 的曲线下面积为 0.79,显着大于 MPO-ANCA(0.58;P< 0.001)。使用ROC曲线确定的 0.8 U/mL 截断值,PR3-ANCA 显示出比 MPO-ANCA显着更高的灵敏度(64.9%) 和更好的特异性 (83.6%)。
