Neurology:中枢神经网络连接增加与多发性硬化认知障碍

重度认知障碍的多发性硬化患者,其默认模式网络和额顶网络与大脑的其余部分连接增加导致信息处理相互干扰引发认知能力降低

近日研究人员探讨认知障碍(CI)多发性硬化症(MS)患者中与认知和大脑神经功能有重要关系的默认模式网络(DMN)和额顶网络(FPN)功能的改变。

研究人员对322名多发性硬化患者以及96名健康对照者进行静息状态fMRI与神经心理学评估,多发性硬化患者依据Z分数分类:重度认知障碍(CI组,至少2试验Z评分<-2;n = 87),中度认知障碍(至少2测试Z评分<-1.5;n = 65)以及认知保留(CP n = 180)。

研究发现,相比于健康人群与CP组,仅CI组患者DMN以及FPN与大脑的其余部分(下顶叶、扣带回后部驱动和角回)连接增加,但二者内部或之间的连接并无变化。与其他大脑的连通性增加却导致患者认知能力降低,这表明认识功能不仅与参与功能的大脑结构单元数量相关更重要的是各个单元间对信息的处理的独立性。

研究表明,对于重度认知障碍的多发性硬化患者,其默认模式网络和额顶网络与大脑的其余部分连接增加导致信息处理相互干扰引发认知能力降低。
