Circulation:碾碎服用是否可提高普拉格雷对心肌梗死患者的疗效?

专家建议,拟行经皮冠状动脉介入治疗的ST段抬高型心肌梗死患者,可在pPCI前服用P2Y12抑制剂,本研究旨在评估ST段抬高型心肌梗死患者服用整片或碾碎的普拉格雷对冠脉再灌注的影响。

经皮冠状动脉介入治疗(PCI)是ST段抬高型心肌梗死患者的首选治疗方案,可使梗阻血管再通,恢复微循环灌注,从而减少心肌损伤。专家建议,拟进行首次经皮冠状动脉介入治疗(pPCI)的ST段抬高型心肌梗死患者,可在进行pPCI之前尽早予以P2Y12抑制剂口服治疗。将P2Y12抑制剂碾碎后服用,可促进更快更有效的发挥血小板抑制作用,但服用碾碎的P2Y12抑制剂对冠脉再灌注的影响尚不明确。

本研究是一项随机对照试验,招募拟进行pPCI的ST段抬高型心肌梗死患者,在救护车转运之前就予以整片碾碎的普拉格雷(60 mg)。

独立的主要终点是初次冠脉造影时梗死相关动脉TIMI3级血流(即完全再灌注)和pPCI后一小时ST段的恢复情况。安全性终点是3级及以上大出血(TIMI和BARC评估标准)。次要终点包括血小板反应性和缺血预后。

共727位患者被分至两组,服用了整片或碾碎的负荷剂量的普拉格雷。碾碎组和整片组患者在pPCI之前主要终点梗阻相关动脉TIMI3级血流率分别为31.0% vs 32.7%(优势比 0.92,95% CI 0.65-1.30,p=0.64),pPCI后一小时ST段完全恢复率分别是59.9% vs 57.3%(优势比 1.11,95% CI 0.78-1.58,p=0.55)。

pPCI开始时,两组间的血小板反应性(以P2Y12反应单位计算)存在显著差异(碾碎组 192 [132–245] vs 整片组 227 [184–254], p≤0.01)。碾碎组和整片组TIMI标准的≥3级出血的发生率分别是0% vs 0.8%,BARC标准的分别是0.3% vs 1.1%。此外,两组30天内的缺血事件发生情况未见差异。

综上所述,对于拟行pPCI的ST段抬高型心肌梗死患者,院前予以服用碾碎的普拉格雷片并不能提高其pPCI前梗死相关动脉TIMI3级血流再通或pPCI后一小时ST段完全恢复的可能性
