镰状细胞性贫血(SCA)是全世界最常见的单基因疾病。该疾病与神经系统并发症有关,包括卒中、无症状脑梗塞(SCI)和认知障碍,所有这些并发症都可能显著影响社会和经济流动性以及患者的生活质量。最近的研究证据表明脑血流动力学压力在SCA患者神经系统并发症发生发展中发挥这重要作用,但脑血流动力学机制相对复杂,并且仍未明确。

先前的研究已经使用动脉自旋标记(ASL)定性描述了SCA患者的高静脉信号,与动静脉分流的信号一致。近日,脑血管和代谢疾病权威杂志Journal of Cerebral Blood Flow & Metabolism上发表了一篇研究文章,研究人员旨在量化影响并探索与动脉氧含量(CaO2)、疾病缓解治疗、无症状脑梗死(SCI)和认知表现之间的横截面关联。

94名SCA患者和42名对照组接受了单次和多次流入时间(TI)ASL序列的认知评估和MRI检查。在矢状窦的灰质和白质以及高信号区域检查脑血流量(CBF)和推注到达时间(BAT)。在灰质和白质中,研究人员观察到CBF增加和BAT减少与患者CaO2减少相关,而与序列无关。

在高信号矢状窦区域,CBF也随着使用这两种序列的CaO2减少而增加。然而,在这些区域的患者中,BAT增加而不是减少,与CaO2无关。在患者中使用multiTI序列,白质和高信号矢状窦区域的CBF增加与较差的认知能力相关。

这些新发现突出了multiTI评估的ASL在阐明和识别SCA患者局部血流动力学压力的客观量化和功能标志物方面的效用。
