甲状旁腺或血管操作引起的短暂性低钙血症是甲状腺切除术后常见的并发症。考虑到25-羟维生素D(25(OH)D)在止血钙中的作用,研究人员进行了一项随机临床试验,评价了甲状腺切除术患者术前补充维生素D对低钙血症发生率的影响,研究结果已发表于Am J Med Sci。

研究纳入了100名计划行甲状腺全切除术并患有中度或重度维生素D缺乏症的患者。将患者随机分到研究组或对照组,研究组患者在术前4周内每周接受维生素D3 5万珠。对照组接受安慰剂。术前、术后当天、术后2周检测两组患者的总血清和游离血清钙水平。

 

结果显示,在人口统计学数据方面,两组之间没有显著差异。在连续的总血清钙水平检测(5次)中,与对照组相比,维生素D补充剂组发生了显著变化(P=0.043)。补充25-羟基维生素D(25(OH)D)的患者症状性低钙血症的发生率明显降低(P = 0.04)。同时,与对照组相比,试验组为了治疗低钙血症症状而静脉注射钙剂的要求明显降低(P = 0.03)。

综上所述,该研究结果表明,维生素D缺乏症患者补充维生素D会降低早期症状性低钙血症的发生率,相应的,为治疗低钙血症而补充钙的需求较低。
