美国西拉斐特普渡大学药剂学系教授、该AHA科学声明撰写委员会主席James E Tisdale教授指出,许多常用药物都会导致心跳不规则,这是一种副作用。虽然风险相对较低,但医护人员必须考虑到,患者的心律失常可能是由药物引起或加剧的。

该声明回顾了可能导致或加剧心律失常的药物、相关的危险因素以及有心律失常风险或发生心律失常人群的预防、监测和治疗方案。

COVID-19大流行期间,氯喹、羟化氯喹和阿奇霉素被用于治疗这种新型冠状病毒疾病。这些药物会引起心律失常,已经有一些临床试验来评估它们治疗COVID-19的有效性,其中包括一项在6月份停止的美国国立卫生研究院(National Institutes of Health)试验。6月和7月,美国食品和药物管理局(FDA)撤销了紧急使用,并发出警告,禁止在临床试验或医院外使用羟化氯喹或氯喹治疗COVID-19。

AHA在声明中指出,有心脏病发作史、心脏病史或心脏手术史的人在接触某些药物后更容易出现药物相关心律失常。药物诱发心律失常的其他危险因素包括年龄较大、钾或镁缺乏以及过量饮酒。那些多次发生药物性心律失常的患者可能有遗传倾向。诊断心律失常最常见的检测方法是心电图(ECG)。

遵医嘱服药并保持正常的电解质水平、肾和肝功能有利于降低风险。其他预防策略包括使用诱发心律失常药物的最低有效剂量、尽量减少或避免使用兴奋剂以及避免过度饮酒(女性:每天一杯或更少;男性:每天两杯或更少)。了解和避免药物相互作用也有助于将风险降至最低。

对于某些高风险药物,患者可能需要在开始用药时住院并使用心脏监护仪。对于药物性心律失常风险增加的患者,也可进行定期心电图监测。

治疗通常包括停止服用引起心律失常的药物,也可能包括抗心律失常药物,或放置一个可以纠正不规则心跳的装置。同时,Tisdale教授强调,药物对于治疗各种疾病和慢性病极其重要和有益,患者不应在未与医疗专业人员交谈前改变或停止服用任何药物。
