最近,出现了新的数据驱动的疾病进展建模方法,聚焦于生物标志物变化的级联。基于事件的模型是一类疾病进展模型,用于估计来自横断面数据的生物标志物变化的级联。这是在没有关于不同生物标志物之间关系的强先验假设的情况下完成的。估计生物标志物变化级联的一种有前途的新方法是基于事件的判别模型(DEBM)。该模型对队列中的疾病表型异质性具有鲁棒性,并且可以处理缺失数据。在这项研究中,使用DEBM来评估症状前和症状性FTD-GRN突变携带者中生物标记物变化的时间级联,区分早期和晚期生物标记物。此外,本文还测定了nfvPPA和bvFTD中生物标志物变化模式的表型差异,以进一步了解其独特的疾病进展机制。

受试者来自遗传性额颞叶痴呆(GENFI)的三个欧洲中心:鹿特丹(荷兰)、布雷西亚(意大利)和巴塞罗那(西班牙)。收集了126名参与者的认知和临床数据、核磁共振成像和血清样本。包括35例症状性GRN突变携带者(鹿特丹:n=11,布雷西亚:n=22,巴塞罗那:n=2),56例症状前GRN突变携带者(鹿特丹:n=33,布雷西亚:n=17,巴塞罗那:n=6)和35例认知健康非携带者(鹿特丹:n=34,布雷西亚:n=0,巴塞罗那:n=1)。当地临床遗传学部门进行了DNA基因分型,以确认是否存在GRN突变。非携带者是GRN患者的一级家庭成员,无突变。有症状的突变携带者根据已建立的bvFTD16(n=17)、nfvPPA17(n=16)或皮质基底综合征18(n=2)的临床标准进行诊断。突变携带者在不符合临床标准时被定义为症状前,即没有行为或认知症状。19份临床调查问卷被发放给照顾者、配偶或家庭成员,即额颞叶变性临床痴呆评定量表(FTD-CDR-SOB),神经精神量表(NPI)和额颞叶痴呆评定量表(FRS)。该研究是根据赫尔辛基宣言进行的,由每个地点的当地医学伦理委员会批准,所有参与者提供书面知情同意书。

对于生物标志物的选择,使用Pubmed进行了文献检索。研究包括:(1)对症状前GRN突变携带者进行研究;(2)对血液或脑脊液(CSF)中的生物标志物、神经影像学生物标志物和认知进行研究。选取了血清NfL、9项简易精神状态检查(MMSE)、注意力和处理速度的认知领域、执行功能、语言和社会认知;脑岛、额叶、顶叶和颞叶的左右灰质体积。通过静脉穿刺获得血清样品,并用单分子分析技术进行分析。10个样品在单个实验室中测量,一式两份,分析内变异系数低于5%。批间分析差异低于8%。NfL浓度以pg/mL表示。利用3TMRI扫描仪对三个部位进行三维T1加权和弥散张量成像。所有选定的生物标志物均进行了正态性测试,并在出现偏态分布的情况下进行对数转换。对于每个生物标志物,它首先使用高斯混合模型(GMM)估计正常值和病理(或异常)值的分布,并使用这些来计算每个受试者的生物标志物异常的概率。对于成像生物标志物,修改了DEBM中的GMM步骤,使其更适合于FTD-GRN人群,因为其不对称的萎缩模式。与早期生物标志物相比,通常在疾病晚期变得异常的生物标志物的异常值在特定患者人群中通常表现不足。

结果显示了生物标志物变化的估计平均级联以及非成像和多模式生物标志物模型内的不确定性。语言是最早出现异常的生物标志物,其次是NfL。左前丘脑辐射、左岛叶和双侧钩束是最早的影像学生物标志物。还可以观察到,左半球的成像生物标志物比右半球的成像生物标志物更早出现异常。疾病严重程度将有症状的受试者与症状前受试者区分开来。使用非成像生物标志物时,这种描绘的敏感性和特异性分别为1.0和0.982;使用多模式生物标志物时,这种描绘的敏感性和特异性分别为1.0和0.961。nfvPPA和bvFTD受试者的估计疾病严重程度与症状出现后的年数和FTD-CDR-SB的相关性。在nfvPPA患者中,估计的疾病严重程度与症状出现后的年数(R=0.95,p=0.0003)和FTD-CDR-SB(R=0.84,p=0.0189)密切相关。然而,在bvFTD患者中,估计的疾病严重程度与症状出现后的年数(R=0.22,p=0.6331)和FTD-CDR-SB(R=0.28,p=0.5866)相关性较差。

nfvPPA患者首先表现为语言和NfL异常,其次是其他认知领域。左半球成像生物标志物在右半球成像生物标志物之前变得异常,首先是无节束(白质完整性)、岛叶和颞叶(灰质体积)。只有左上纵束被认为是晚期生物标志物,甚至比右上纵束晚。bvFTD患者中,生物标志物排序也表明语言和NfL是最早的异常生物标志物。与nfvPPA相比,左上纵束(白质完整性)被认为是bvFTD的第一个异常影像生物标志物。然而,bvFTD中的生物标志物顺序主要以疾病时间轴中生物标志物定位的巨大不确定性为特征,早期和晚期生物标志物之间几乎没有任何可观察的区别。

通过FTD-GRN谱中的DEBM研究,能够证明语言功能和NfL水平是最早的异常生物标志物。然而,bvFTD在疾病进展中表现出更多的异质性和不确定性,表明生物标志物比nfvPPA具有更多的变异性。分析表明,轴突变性和语言网络损伤是GRN突变携带者最早的生物标志物,有可能作为疾病治疗临床试验的终点。未来的工作应该是用纵向数据来确认和验证这些发现。
