传统的团体医疗访视往往通过强化用药以改善糖尿病患者的血糖水平。相比之下,以体重控制为主导的团体医疗访视获益情况如何?那么,以体重控制主导的团体医疗访视能否获得同样的获益呢?据近日发表于JAMA内科杂志的研究结果,以低碳水化合物饮食控制体重的团体医疗访视可有效降低血糖水平、减轻体重,同时减少降糖药使用。

传统的团体医疗访视往往通过强化用药以改善糖尿病患者的血糖水平。相比之下,以体重控制为主导的团体医疗访视获益情况如何?那么,以体重控制主导的团体医疗访视能否获得同样的获益呢?据近日发表于JAMA内科杂志的研究结果,以低碳水化合物饮食控制体重的团体医疗访视可有效降低血糖水平、减轻体重,同时减少降糖药使用。

研究概述

这项随机、对照、非劣性研究比较了以药物强化为主的团体医疗访视项目和以低碳水化合物饮食控制体重为主的团体医疗访视项目管理糖尿病的效果。此研究在美国北卡罗莱纳州的两个城市开展,研究者随机将263例受试者(平均年龄60.7±8.2岁)分配至以药物强化为主的团体医疗访视组(GMV组)(136例)和以低碳水化合物饮食控制体重的团体医疗访视组(WM/GMV组)(127例)。GMV组:前16周每4周随访1次,后续每8周随访1次,共计随访9次,主要内容为糖尿病自我管理和强化用药。WM/GMV组:前16周每2周随访1次,后续每8周随访1次,共计随访13次,内容包括低碳水化合物饮食、体力活动和体重管理咨询及常规用药。

主要结果:HbA1c

基线时,两组平均HbA1c水平分别为9.0%±1.3%、9.2%±1.3%;平均BMI分别为35.6±5.4kg/㎡、35.0±4.8kg/㎡。研究结束时(48周),两组HbA1c水平均有改善,WM/GMV组HbA1c水平降低0.9%,GMV组HbA1c降低0.8%,平均差异为0.1%(P=0.44,95%CI:0.2~0.5)。较GMV组,16周时WM/GMV组HbA1c水平多下降0.7%(95%CI:0.4~1.0),32周时 WM/GMV组HbA1c水平多下降0.5%(95%CI:0.8~1.0)。

次要结果:低血糖事件、用药情况、体重减轻

研究期间,WM/GMV组低血糖事件发生次数3.2次,GMV组低血糖事件发生次数6.6次,WM/GMV组低血糖事件发生频率降低近一半(RR=0.49;95%CI:0.27~0.71)。WM/GMV组药物疗效评分(MES)从基线2.3分降低至2.2分,而GMV组上升至2.7分,组间差距为0.5分(P<0.001, 95%CI:0.3~0.6)。WM/GMV组体重下降4.1kg,GMV组下降0.4kg,组间差距为3.7kg(P<0.001, 95%CI:1.9~5.5)。

研究者述评

杜克大学医学系William S. Yancy教授表示,“体重管理策略在减重、减少降糖药使用以及减少低血糖事件方面更为有效。既往研究显示与传统一对一访视相比,团体访视能够为患者带来更多获益。这可能是在团体访视中,患者之间可以共享一些想法并且互相之间获得支持;同时,临床医生能够从团体访视中患者的互动中获得更大的满足感。此项研究结果支持了基于体重管理的团体医疗访视会带来一些额外的获益。”
