SARS-CoV-2感染强化了高Lp(a)水平与IHD之间的相关性，但Lp(a)不影响血栓栓塞事件的风险。

包括缺血性心脏病(IHD)在内的合并症加重了SARS-CoV-2感染后的病情。较高的脂蛋白(a) [Lp(a)]水平是IHD和血栓栓塞事件的一个强大危险因素。

近日，内科学领域权威杂志Journal of Internal Medicine上发表一篇研究文章，研究人员旨在评估SARS-CoV-2感染是否改变了高Lp(a)浓度在预测IHD或血栓栓塞事件方面的风险。

研究人员利用英国生物样本库对SARS-CoV-2大流行期间的数据进行队列研究，并比较了SARS-CoV-2阳性患者和人群对照组的基线Lp(a)水平。英国生物样本库获得了西北多中心研究伦理委员会的伦理批准(REC:11/NW/0382)。根据《赫尔辛基宣言》的原则，所有参与者在参与研究前均给予书面知情同意。

Lp(a)、SARS-CoV-2、IHD和血栓栓塞事件的关系

SARS-CoV-2阳性患者Lp(a)浓度与对照组相似。在人群对照者(n=435104)和SARS-CoV-2阳性患者(n=6937)中，随着Lp(a)浓度的升高，IHD的风险增加。Lp(a)的遗传风险评分支持了这些结果之间的因果关系。SARS-CoV-2感染改变了这种关联，感染患者的风险显著增加(交互作用p=0.03)。尽管SARS-CoV-2阳性患者发生血栓栓塞事件的频率比对照组高出5倍(1.53% vs. 0.31%)，但Lp(a)对风险没有影响。

由此可见，SARS-CoV-2感染强化了高Lp(a)水平与IHD之间的相关性，但Lp(a)不影响血栓栓塞事件的风险。
