中医推拿解决临床“筋骨病疼痛”问题特色鲜明,疗效显著。我院推拿学科带头人、推拿研究所所长房敏教授团队在前期推拿镇痛研究基础上,首次探索了推拿通过调控外周神经炎性微环境发挥抗炎镇痛效应的miRNA参与机制,研究结果发表在Journal of Neuroinflammation(IF: 9.59)杂志。

随着现代人们生活方式的改变,腰椎间盘突出症(LDH)的发病人数日渐增长并且趋于年轻化,严重降低了人们的生活质量,其中LDH诱发的外周神经局部炎性微环境可能是疼痛发展过程中的关键驱动因素

推拿作为中医传统疗法的一种已经应用了几千年,《医宗金鉴·正骨心法要旨》曰:“气血郁滞,为肿为痛,宜用按摩法。按其经络,以通郁闭之气,摩其壅聚,以散痰结之肿,其患可愈”,表明推拿可以通过疏通经络,散结消痰,从而“通郁散结”起到镇痛的功效。

即使在几千年后的今天,推拿依然在临床中用于解决疼痛问题,目前已成为专家共识所推荐的干预LDH有效方法之一,造福了万千患者。团队过去致力于从临床研究角度,通过生物力学方法证实了推拿疗法的安全性和有效性,取得了一定的理论与实践成果;而在基础研究中,团队也证实了推拿的镇痛作用可能与改善机体局部炎症变化有关。但是,这些因子发生变化的分子机制及推拿抗炎镇痛的机理尚不明确,有待于进一步研究。团队近年来潜心研究,阐释了miRNA在介导推拿发挥抗炎镇痛中的重要启动作用,证实了推拿可能通过miRNA调控炎症相关因子表达,从而改善外周神经局部炎性微环境。

2022年9月,上海市中医药研究院中医推拿研究所房敏教授团队针对推拿镇痛效应机制发表了相关研究结论,题目为Transcriptome profiling of microRNAs reveals potential mechanisms of manual therapy alleviating neuropathic pain through microRNA-547-3p mediated Map4k4/NF-κb signaling pathway。

该研究首次运用miRNA高通量测序技术研究了推拿按揉对LDH模型大鼠抗炎镇痛效应的潜在机制,阐释了在推拿手法干预外周神经炎性微环境发挥抗炎镇痛效应过程中miRNA是其中重要启动因素,凸显了推拿作为中医传统非药物疗法在未来疼痛相关疾病治疗中的独特优势。

全文解读

研究人员首先通过采用背根神经节持续压迫(CCD)大鼠模型来模拟LDH的病理变化。模型验证成功后,通过佩戴手指触觉测量指套来固定推拿按揉的刺激量,对“委中穴”(BL40)进行为期天的干预,18 证实了推拿的抗炎镇痛效应

近年来随着疼痛相关研究的深入,发现微小RNA(miRNA)在细胞中具有特定的调控功能,可以抑制同源靶基因的表达,从而对机体的疼痛传导产生影响。作为中医传统非药物疗法,推拿疗法的多途径、多靶点的干预方式与miRNAs的作用方式高度契合

因此研究人员进一步以miRNA作为切入点,基于miRNA-seq高通量测序技术,研究大鼠背根神经节组织(DRG)中差异表达miRNAs与LDH外周神经损伤的关系,证实了推拿可以通过调控miRNAs来发挥抗炎镇痛作用

为了找到介导推拿发挥抗炎镇痛效应的关键miRNA,研究人员进一步从差异表达的miRNA中进行筛选与生物信息学分析,最终找到了miR-547-3p以及其靶基因Map4k4,并通过体外细胞实验证实了miR-547-3p对Map4k4的调控作用。

最后,该研究进一步分析了miR-547-3p可能的效应途径,发现其可能通过靶向Map4k4调控下游炎症相关的NF-κB信号通路。因此,研究人员通过经化学特殊修饰的寡核酸合成物agomir和antagomir对miR-547-3p进行过表达或拮抗,检测各组大鼠DRG组织中炎症因子表达情况和NF-κB通路相关p65、IκBα的表达和磷酸化水平,证实了推拿按揉可能通过miR-547-3p介导的Map4k4/NF-κB通路影响下游炎症因子来发挥抗炎镇痛效应

本研究进一步以miRNA作为切入点证实了外周神经局部炎症与疼痛之间的相关性,丰富了疼痛相关理论,阐释了中医“聚沫为痛”的理论内涵。率先运用miRNA-seq高通量测序技术探索介导推拿发挥镇痛效应的miRNAs及其机制,阐释了在推拿手法干预外周神经炎性微环境发挥抗炎镇痛效应过程中miRNA是其中重要启动因素,是对当前推拿镇痛机制的全新探索,也为进一步临床研究开拓了新的方向。

团队将充分基于当前研究基础,继续探索推拿“外治内应”的效应机制来丰富中医理论并应用于实践,为进一步解决临床实际问题提供具有中医特色的科学诊疗方案

本研究作为中医推拿疗法在国外高质量SCI论文中的一次亮相,本文作者对“推拿”进行了一次较为详细的论述。

手法治疗(MT)是许多国家传统医学的重要组成部分,数千年来,由于其在临床治疗(特别是缓解疼痛)中的疗效,在中国得到了广泛应用。类似于针灸,中国的MT主要基于中医传统经络腧穴理论,通过刺激体表的各种反应点来发挥治疗作用。然而,与针灸不同的是,即使对于同一点,MT也有不同的操作手法,比如按法、点和揉等;因此,它在中国被称为推拿(Tuina, pressing and holding”。

该论述向国外读者介绍了中医推拿疗法的基本概念与特色,引起了审稿专家的强烈兴趣,并在审稿过程中与作者进行了探讨,这也为推拿进一步“走出去”奠定了基础

值得关注的是,2022年房敏教授带领的中医推拿传承创新团队已经在Journal of Neuroinflammation (IF:9.59)、Frontiers in Public Health(IF: 6.42)、Frontiers in Aging Neuroscience(IF: 5.70)、Frontiers in Psychiatry(IF: 5.44)、Frontiers in Medicine(IF: 5.06)、American journal of translational research(IF: 3.94)、Frontiers in Neurology(IF: 4.09)等杂志中发表10SCI论文

以上研究成果为推拿(手法和功法)提供有力的基础与临床研究证据支持,也更好地促进传统中医非药物疗法推广与应用。团队将继续在房敏教授的带领下,砥砺前行,努力推动学科发展。
