多次脉冲射频治疗臂丛神经损伤后疼痛1例

外周神经损伤引起的神经病理性疼痛属于神经损伤常见并发症,其产生的机制复杂,病人常伴有抑郁焦虑,治疗效果差是困扰临床医生的一个问题。射频技术在治疗外周神经损伤引起的疼痛方面取得了一定效果,但由于神经损伤的部位难以确定,往往会影响治疗效果。超声引导技术的应用,大大提高了治疗效果。最近我科利用超声引导技术对臂丛神经损伤后疼痛的病人进行多次脉冲射频治疗,效果良好,现报道如下:

外周神经损伤引起的神经病理性疼痛属于神经损伤常见并发症,其产生的机制复杂,病人常伴有抑郁焦虑,治疗效果差是困扰临床医生的一个问题。射频技术在治疗外周神经损伤引起的疼痛方面取得了一定效果,但由于神经损伤的部位难以确定,往往会影响治疗效果。超声引导技术的应用,大大提高了治疗效果。最近我科利用超声引导技术对臂丛神经损伤后疼痛的病人进行多次脉冲射频治疗,效果良好,现报道如下:
 
曾某某,男,56岁,二十年前因车祸伤在外院行右侧臂丛神经损伤修复手术,术后恢复尚可,后病人出现右上肢疼痛,疼痛呈刺痛,伴有麻木,后逐渐出现上肢肌肉萎缩,疼痛逐渐加重。口服美洛昔康,塞来昔布,曲马多及加巴喷丁等药物,疼痛控制差,体格检查:右上肢肌肉萎缩,肌力1级,感觉减退,视觉模拟评分(visual analogue scale,VAS):8分,焦虑自评量表(self-rating anxietyscale,SAS):4分,超声下看到臂丛神经粘连严重,解剖不清晰(见图1)。

治疗经过:予病人口服美洛昔康7.5mg,每日1次,羟考酮1mg,每日1次,加巴喷丁0.3g,每日3次。于超声引导下行脉冲射频治疗(见图2)。

在超声引导下穿刺到臂丛神经位置穿刺针型号,使用华夏康宁射频仪(安科ASA-601TP射频仪)进行神经阻抗测试,阻抗在380欧姆,进行神经感觉和运动测试,进一步确定在神经周围,采取标准脉冲射频,温度42℃,时间360s,射频结束后局部注射复方倍他米松1.5mg,甲钴胺0.5mg,2%利多卡因1ml,拔除射频针。第二天病人诉拇指部位疼痛明显减轻,VAS评分6分,间隔五天行第二次超声引导下脉冲射频治疗,温度42度,时间360秒,射频治疗后病人诉小手指疼痛减轻,VAS评分4分。
 
停用羟考酮,病人疼痛评分维持在3~5分左右,随后行第三次超声引导下脉冲射频治疗,方法同第二次,病人疼痛明显减轻,VAS评分2分,停用美洛昔康。观察三天,病人VAS评分1~3分,SAS评分1分。停用加巴喷丁,病人出院。病人一月后疼痛再次出现,来院求治,VAS评分7分,病人再次行超声引导下脉冲射频治疗,治疗方法同第二次,病人治疗结束后VAS评分在5分,后续进行射频治疗7次,病人共计脉冲射频治疗11次。病人VAS评分逐渐降低到1分。对此病人随访3月,病人VAS评分0~1分。
 
臂丛神经损伤是临床上常见的神经损伤。外周神经损伤后损伤区域的神经元异常放电引起外周神经敏化从而引起疼痛。神经损伤病人往往长期伴有神经病理性疼痛。神经损伤后神经病理性疼痛是临床上常见的疼痛常伴有不同程度的抑郁。由于神经损伤疼痛的机制复杂,临床上用于治疗神经病理性疼痛的药物很多,但单纯药物治疗效果欠佳。
 
超声影像下可以看到神经损伤的部位出现异常信号,研究表明超声技术用于神经筋膜损伤诊断要优于核磁共振。在超声下看到病人臂丛神经周围粘连严重,臂丛神经周围瘢痕增生,局部炎症反应明显,也许是病人产生神经病理性疼痛的原因之一。超声引导技术促进了疼痛微创治疗的开展,在超声引导下穿刺臂丛神经,并且根据感觉和运动电刺激的结果调节针尖在臂丛神经的不同部位,达到精准治疗的目的。
 
脉冲射频治疗疼痛在临床上广泛使用,其发出高频交流电,针尖温度不超过42度,在神经周围形成高电压,从而起到调节疼痛的作用。脉冲射频治疗神经损伤性疼痛的效果有不少报道,肖林研究表明脉冲射频治疗神经损伤性疼痛有明确的临床效果。采用脉冲射频方式也取得良好的效果。但是,单次脉冲射频虽然可以调控疼痛,其调控疼痛时间不长。此病人臂丛神经损伤近二十年,传统治疗方法基本没有效果。
 
在超声引导下进行脉冲射频治疗后病人镇痛评分明显下降,和前人研究一致,脉冲射频可以治疗外周神经损伤性疼痛,此病人进行脉冲射频三次才取得较好的临床效果,可能是因为此病人臂丛神经损伤周围瘢痕增生明显影响了脉冲射频治疗效果。Endine研究表明,连续十天在小鼠坐骨神经进行脉冲射频,小鼠的神经超微结构有明显变化。此病人共使用脉冲射频治疗11次,随访3月病人基本在无痛状态,表明脉冲射频在神经病理性疼痛治疗过程中可以起到神经调控作用,持续的脉冲刺激是否可以改变神经可塑性变化,这是值得商榷的问题,但是经过十一次脉冲射频治疗确实起到了十分良好的作用,有关机理值得进一步探讨。
