        质子泵抑制剂(PPI)是全球十大最常用的药物之一。PPI通常用于与酸有关的疾病,例如胃食管反流疾病,消化性溃疡疾病和非溃疡性消化不良。一般认为,短期使用PPI是安全的。但是,长期使用PPI已与各种不良反应有关,例如骨折,慢性肾脏疾病,肠感染胃癌。另外,2型糖尿病已成为全球流行病,2014年全球患病率为8.5%,2型糖尿病的病因很复杂,涉及多种遗传,行为和环境因素。近年来,研究人员将注意力转向了人类肠道菌群的作用,这对于扩大宿主代谢过程在糖尿病发展中的作用至关重要。鉴于PPI对肠道微生物组的显着影响,其使用可能与2型糖尿病的风险增加有关。这项研究旨在评估PPI的常规使用和2型糖尿病的风险。

 

        研究人员对204689名无糖尿病的参与者进行了回顾性研究。使用美国糖尿病协会(ADA)的诊断标准确认了2型糖尿病的发病情况。然后评估了人口因素,生活方式习惯,合并症,其他药物的使用对2型糖尿病发病风险的影响。

 

        本项研究共记录了10105例糖尿病病例,在随访了2127471人年后,普通PPI使用者患糖尿病的风险比非使用者高24%(HR 1.24,95%CI 1.17至1.31)。服用PPI的持续时间会增加糖尿病的风险。与非使用者相比,使用PPI大于2年的参与者的完全调整后的HR为1.05(95%CI 0.93至1.19)。

 

         因此,研究人员证实定期使用PPI与2型糖尿病的风险较高有关,并且随着使用时间的延长,风险会增加。因此,医师在开处方PPI时应格外小心,尤其是长期使用时。
