Heart:生长分化因子-15作为肺动脉高压成人死亡率的预测因子

由此可见,高GDF-15水平与PH成人死亡或移植风险增加有关,与年龄和NT-proBNP无关。作为非特异性生物标志物,GDF-15可用于检测低风险患者。

尽管生长分化因子-15(GDF-15)对各种疾病的死亡率具有预测价值,但它作为肺动脉高压(PH)的预后生物标志的相关性仍不清楚。近日,心脏病领域权威杂志Heart上发表了一篇研究文章,该研究旨在调查GDF-15与PH成年患者预后之间的关系。

这是一项单中心前瞻性观察性队列研究。所有在2012年至2016年期间进行诊断性右心导管检查的PH成年患者检查当天被纳入研究。排除了由于左心脏病引起的PH。进行静脉血液取样并检测GDF-15和N-末端pro-B型利尿钠肽(NT-proBNP)水平。 Kaplan-Meier曲线和Cox回归分析用于研究GDF-15与死亡或肺移植的复合终点之间的关联。研究人员在多变量分析中调整了年龄和NT-proBNP。通过测量健康对照者GDF-15水平建立参考值。

研究人员测量了103名患者GDF-15水平(中位年龄为59.2岁,65%为女性,51%肺动脉高血压)。76名患者GDF-15水平升高(74%)。中位随访3.4(IQR为2.3-4.6)年后,32名患者(31.1%)发生主要终点。GDF-15正常的患者诊断后2年无事件生存率为100%,而GDF-15升高的患者为72.4%(p=0.007)。GDF-15与主要终点之间存在显著相关性(升高2倍的HR为1.77,95%CI为1.39-2.27,p<0.001),同样在调整年龄和NT-proBNP后(HR为1.41,95%CI为1.0-1.94,p=0.038)。

由此可见,高GDF-15水平与PH成人死亡或移植风险增加有关,与年龄和NT-proBNP无关。作为非特异性生物标志物,GDF-15可用于检测低风险患者。
