nature子刊:发现肠道菌群代谢物促进心脏肥大!

研究发现菌群活性代谢产物TMAVA通过抑制脂肪酸氧化加重高脂饮食诱导的心脏肥大。

心力衰竭 (HF) 是一个全球性问题,全世界估计有3800万患者患有这种疾病1。心肌肥大是心力衰竭过程中的早期病理过程,也是心脏疾病发病率和死亡率的重要危险因素2。深入对心脏肥大病理生物学的理解和探索预防心脏肥大和心力衰竭的新靶点具有重要意义。活性代谢产物是在生物体新陈代谢过程中转化的化学实体,可以反应细胞和机体的功能状态和改变。代谢组学通过进行全局代谢物分析,为探究代谢改变与细胞表型之间的关系提供了窗口。

2022年4月1日,北京大学基础医学院心血管研究所郑乐民团队与北医三院血管所赵明明博士后在Nature子刊Nature Communications在线发表了题为“Gut microbiota production of trimethyl-5-aminovaleric acid reduces fatty acid oxidation and accelerates cardiac hypertrophy”的研究论文,研究发现菌群活性代谢产物三甲基-5-氨基戊酸(TMAVA)通过抑制脂肪酸氧化加重高脂饮食诱导的心脏肥大。

该团队通过对7年随访的心力衰竭患者队列(N=1647)血浆进行三甲基-5-氨基戊酸 (TMAVA)靶向代谢组学检测发现,随着TMAVA水平的升高,心脏移植和死亡的发生率逐渐升高,校正传统风险因素后,TMAVA仍然具有统计学意义。

研究团队为了进一步在动物水平探究TMAVA导致心脏肥大的机制,在高脂饮食(HFD)的基础上,发现给予TMAVA刺激后,小鼠心脏明显肥大,并加剧了心脏功能障碍。机制上,研究团队发现TMAVA不仅通过丁基甜菜碱羟化酶(BBOX)抑制内源性肉碱的合成3(Gastroenterology,2020),同时通过肉碱/有机阳离子转运体(OCTN2)抑制肉碱的摄取,导致血浆和心脏组织中肉碱缺乏。肉碱缺乏抑制脂肪酸氧化,加重高脂饮食诱导的心脏脂质堆积,导致线粒体结构和功能紊乱。进一步,研究团队给予BBOX敲除小鼠,发现BBOX敲除小鼠血浆和心脏组织中肉碱水平下降,在高脂饮食下,油红O和脂质组学提示心脏存在异常的脂质堆积,同时表现出与TMAVA刺激相似的心脏肥大表型,说明BBOX通过抑制肉碱的合成加重心脏肥大,外源性肉碱补充剂可逆转 TMAVA 诱导的心脏肥大。因此,TMAVA、BBOX可能是与肠道菌群相关的干预心脏肥大的潜在靶点。
