AJKD:肾脏组织病理学与肾衰竭预测

将肾活检标本的组织病理学特征纳入预测模型可改善对肾衰竭的预测,在临床上可能有一定价值。需要进一步的研究以了解更详细的肾脏组织特征是否可以进一步改善对估计肾小球滤过率未来变化轨迹的预测。

通过肾脏组织病理学能否预测肾衰竭尚未明确。近日,肾脏病学权威杂志Am J Kidney Dis上发表了一篇研究文章,研究人员假设使用肾脏活检标本的组织病理学特征可改善仅根据人口统计学和临床​变量对临床结局的预测能力。

该研究为回顾性队列研究,并开发了一个临床预测模型,参与者为来自印第安纳州活检生物库研究队列的2720名在2002年至2015年间接受了肾脏活检的患者,并至少随访了2年。该研究的预测变量包括人口统计学变量、合并症、基线临床特征和组织病理学特征。结局为发展为肾衰竭的时间,肾衰竭通过持续估计的肾小球滤过率≤10µmL/min/1.73µm2来定义。研究人员采用多变量Cox回归模型进行分析。包括临床和人口统计学变量在内的模型加入了组织病理学特征进行拟合。为了评估加入组织病理学变量的影响,研究人员计算了解释的方差量(r2)和C指数。通过计算每个组织病理变量和所有合并变量的净重分类指数,评估对预测效能的影响。

中位随访时间为3.1年。在活检的5年内,有411名(15.1%)患者出现了肾衰竭。包含人口统计学和临床​变量在内的多变量分析显示,严重的肾小球退化(校正后的HR为2.03;95%CI为1.51-2.03)、严重的间质纤维化和肾小管萎缩(校正后的HR为1.99;95%CI为1.52-2.59)以及严重小动脉透明质酸变性(调整后的HR为1.53;95%CI为1.14-2.05)与主要结局独立相关。在临床模型中添加所有组织病理学变量后,肾功能衰竭的净重分类指数为5.1%(P<0.001),而模型C的完整统计量为0.915。

由此可见,将肾活检标本的组织病理学特征纳入预测模型可改善对肾衰竭的预测,在临床上可能有一定价值。需要进一步的研究以了解更详细的肾脏组织特征是否可以进一步改善对估计肾小球滤过率未来变化轨迹的预测。
