JAHA:颈动脉内膜中层厚度与心脏猝死风险

在两个基于社区的队列中,C‐IMT与SCD风险相关。C‐IMT可用作SCD风险的标志物,并有可能启动早期治疗干预措施以降低风险。

在大多数情况下,心脏猝死(SCD)与严重的冠心病有关。普通人群中颈动脉内膜中层厚度(C-IMT)是否是亚临床动脉粥样硬化的替代指标,是否与SCD风险相关尚未明确。近日,血管疾病领域权威杂志JAHA上发表了一篇研究文章,这项研究旨在调查C-IMT与SCD风险之间的关系。

研究人员共评估了20862名参与者:社区动脉粥样硬化风险(ARIC)研究的15307名参与者和心血管健康研究(CHS)的5555名参与者。在基线时通过超声测量C‐IMT和颈总动脉内膜中层厚度。斑块的有无是由训练有素的研究者判断。

在平均23.5年的随访中,ARIC研究中有569名参与者发生SCD(每千人每年1.81例)。校正传统危险因素和时变调节因素后,平均C-IMT和颈总动脉内膜中层厚度与SCD风险相关:第4四分位数与第1四分位数相比的风险比(HRs)和95%CI分别为1.64(1.15-2.63 )和1.49(1.05-2.11)。

在CHS中,302名参与者在13.1年内发生了SCD(每1000人每年4.64例)。调整后,最大C-IMT与SCD风险相关:第4四分位数与第1四分位数相比的HR(95%CI)为1.75(1.22-2.51)。伴有斑块与SCD风险增加35%相关:ARIC研究中的HR(95%CI)为1.37(1.13-1.67),CHS研究中为1.32(1.04-1.68)。

由此可见,在两个基于社区的队列中,C‐IMT与SCD风险相关。C‐IMT可用作SCD风险的标志物,并有可能启动早期治疗干预措施以降低风险。
