背景

慢性阻塞性肺疾病(COPD)的肺部影像学检查主要为CT或MRI,但尚无研究将两者一同评价。

目的

本研究旨在利用容积匹配CT和极化氦3 (3He) MRI评价有或无COPD的吸烟史患者来建立多参数反应图(mPRM)指标。

材料与方法

本前瞻性研究收集了行MRI和CT检查、完成肺功能检查、量表和6分钟步行测试的患者。利用GOLD标准对疾病状态进行评价。利用MRI和CT匹配指标生成mPRM体素值。利用Kruskal-Wallis和Bonferroni检验评价不同严重程度下的差异,利用SPEARMAN系数评估相关性。

结果

本研究共纳入了175例有或无COPD且有吸烟史的患者。无COPD吸烟组患者正常mPRM体素值所占比例要高(GOLD I,II,III和IV分别为60% vs 37%, 20%, and 7%, P ≤ .001),异常体素所占比例较小,包括细小支气管疾病(CT表现正常,5% vs 6% [无显著性差异], 11%, and 19% [P ≤ .001 ])、轻度肺气肿(CT表现正常, 异常ADC: 33% vs 54%, 56%, and 54%; all P ≤ .001)。正常mPRM指标与第1秒用力呼气量(FEV1) (r = 0.65, P < .001) 、FEV1与用力肺活量比值(r = 0.81, P < .001)以及扩散能力(r = 0.75, P < .001)具有正相关性;其与生活治疗较差(r = -0.48, P < .001)具有负相关。细小支气管疾病、肺气肿的异常mPRM指标与FEV1 (r = -0.65 、-0.42; P < .001和扩散能力(r = -0.53、-0.60; P < .001)具有负相关,与较差的生活质量具有正相关(r = 0.45、r = 0.33; P < .001)。

结论

多参数反应图显示与肺气肿和小气道疾病相关的2个异常的结构-功能结果,其多见于有吸烟史且正常肺功能和CT检查的人中。
