AJRCCM:基质金属蛋白酶-8与肺动脉高压,下一个国自然基金?

肺动脉高压 (PH) 是一种极其严重的心肺系统的疾病,其预后差,尽管经过目前靶向药物治疗,患者五年的生存率在50%左右。肺动脉高压领域急需新的药物靶点,开启治疗的新时代。

众所周知,血管的基质硬化发生在PH的早期,从而促进了肺血管的重构,然而肺动脉高压发病过程中基质力学和成分改变的主要驱动因素是复杂的,并且仍然知之甚少。

近日,由美国学者发现发表于AJRCCM,其:

该研究发现,基质硬化在肺动脉高压发生发展过程中具有重要作用。通过细胞-基质相互作用的机械信号传导在肺动脉高压 (PAH) 的进行性血管重塑中起主要作用。基质金属蛋白酶 8 (MMP8) 是一种间质胶原酶,参与调节肺和全身血管系统的炎症和纤维化,但尚未探索其在 PAH 发病机制中的作用。该研究报告了 MMP-8 在肺血管系统中的新保护作用。与对照组相比,PAH 患者血浆和肺小动脉中 MMP-8 的表达显着增加。小鼠中 MMP-8 的缺乏导致暴露于缺氧后血管重构增加和右心室衰竭,表明 MMP-8 上调可能代表了对致病性 PH 刺激的适应性反应。肺动脉平滑肌细胞 (PASMC) 中 MMP-8 的缺乏会改变基质和整合素的表达,通过 FAK 和 YAP/TAZ 信号传导的促重塑机械转导来增强增殖和迁移。使用 FAK 抑制剂破坏这种机械生物学反馈可改善体外 PASMC 的过度增殖和增强迁移,并改善体内 PH 和血管重塑。数据表明在增强或模仿 MMP-8 活性以破坏 PAH 发生过程中病理机械信号传导方面的治疗潜力。

研究从患者本身生物样本出发,通过动物及细胞水平的多层次验证,代表肺动脉高压领域转化医学的新思维;该研究首先发现PAH 患者血浆和肺动脉中 MMP-8 表达增加,通过PH 实验模型中肺血管系统验证 MMP-8 表达情况;经过转基因MMP-8 基因敲除小鼠在缺氧诱导的PH中会增加死亡率,加剧血管重塑,加重肺动脉高压的严重程度,右心室肥大并在慢性缺氧后损害右心室功能; MMP-8 缺乏主要涉及肺动脉平滑肌细胞增殖和迁移增强,肺动脉平滑肌细胞表明基质蛋白表达改变,导致通过 FAK 增强的整合素信号传导,研究的机制探索将从 FAK 活性的抑制降低了 MMP-8 缺陷型肺动脉平滑肌细胞的血管重塑行为,并减弱了 MMP-8 缺陷型小鼠中缺氧诱导的 PH; 在没有 MMP-8 活性的情况下,增强肺动脉平滑肌细胞增殖和迁移需要 FAK 下游的 YAP/TAZ 激活。

研究的主要结论:

MMP-8 是 PAH 发病过程中肺血管系统中上调的新型保护因子。MMP-8 通过改变 PASMC 中的基质组成和破坏整合素-β3/FAK 和 YAP/TAZ 依赖的机械信号传导来对抗病理机械生物反馈。

 

本研究的发现或许给肺动脉高压的靶向治疗提供新的可能,另外本研究的思路和设计角度,值得学习,尤其对于准备申请国自然基金的朋友,更是值得收藏。
