Stroke:脑出血患者收缩压降低与急性肾损伤

基线血肌酐水平较高和接受较高剂量尼卡地平的脑出血患者发生AKI和肾AE的风险较高。AKI的发生与3个月时较高的死亡率或致残率相关。

近日,血管疾病领域权威杂志Stroke上发表了一篇研究文章,研究人员旨在明确脑出血患者急性肾损伤(AKI)和肾不良事件(AEs)的发生率和预测指标,以及AKI和肾脏AE对患者死亡或残疾的影响。

研究人员分析了一项多中心试验的数据,该试验将1000名初始收缩压≥180mmHg的脑出血患者在症状发作4.5小时内随机分配为标准降压(收缩压目标值为140-179 mmHg)和强化降压(收缩压目标值为140-179 mmHg)。通过系列测量随机分组3天后每日血清肌酐水平来确定AKI情况。

在1000名患者中,分别在149名患者(14.9%)和65名患者(6.5%)观察到了AKI和肾脏AE。在多变量分析中,较高的基线血清肌酐水平(≥110μmol/L)与AKI(比值比为2.4[95%CI为1.2-4.5])和肾脏AEs(比值比为3.1[95%CI为1.2-8.1])相关。静脉内尼卡地平剂量较大的曲线下面积与AKI(比值比为1.003 [95%CI为1.001-1.005])和肾脏AEs(比值比为1.003[95%CI为1.001-1.006])相关。AKI患者在90天时有较高的死亡风险(相对风险为2.6[95%CI为1.6-4.2])和死亡或残疾(相对风险为1.5[95%CI为1.3-1.8]),而在那些肾脏AEs患者中则没有。

由此可见,基线血肌酐水平较高和接受较高剂量尼卡地平的脑出血患者发生AKI和肾AE的风险较高。AKI的发生与3个月时较高的死亡率或致残率相关。
