HLH-2004嗜血细胞淋巴组织细胞病(HLH)诊断指南中列出的血清铁蛋白(SF)水平的阈值具有较低的特异性。本研究旨在评价入院SF在HLH诊断程序中的作用,并为发热的HLH疑似队列找到一个合适的阈值。

 

研究纳入2015年9月1日至2016年7月31日期间北京儿童医院收治的所有发热患者,入院时检测SF。研究SF在HLH诊断中的意义及其与HLH相关临床特征的关系。

 

结果显示,357例患者中,共诊断出39例HLH(24例EBV相关HLH,13例不明病因引发HLH,2例家族性HLH)。入院SF的最佳截止值为934 ng/mL,其敏感性、特异性、阳性预测值(PPV)和阴性预测值(NPV)分别为87.2%、88.4%、47.9%和98.3%。与500 ng/mL相比,新SF标准在HLH诊断中的特异性和PPV分别提高了11.7%和14.0%,表明对 "非HLH "的诊断能力和 "HLH "鉴别的真实性有所提高。在4例入院SF在500 ng/mL~934 ng/mL之间的HLH患者中,有2例患者的HLH诊断得到了其他实验室结果的保证,但其余2例患者可能出现误诊。

 

综上所述,该研究结果表明,入院SF水平的临界值升高似乎更适合于区分发热患者的HLH。诊断时SF水平的确切截止值尚需确定。
