stroke:靶向血浆蛋白组学结果,这几类蛋白可以准确预测颈动脉斑块的发展

这组新的循环蛋白可被视为临床前动脉粥样硬化发展的标志物

初级预防中对心血管进行风险分层是一项临床挑战。最近,有学者发现了一大批循环蛋白,改善了对心血管事件的风险预测。为此,来自意大利米兰大学的研究人员评估这些蛋白质中哪些可以预测初级心血管预防中亚临床颈动脉粥样硬化(SCA)的发展,结果发表在stroke杂志上。

通过近距离延伸检测,对586名以前没有心血管事件和没有临床前动脉粥样硬化的受试者在基础访问时收集的血浆中的368种蛋白质进行了量化。在纵向观察分析中,这些受试者在中位随访11年后被重新评估,以评估SCA的发展,SCA被定义为在任何颈动脉中形成病灶,在基础访问和随访后被颈动脉超声检测到。在相同的随访期间,还通过超声测量颈总动脉(内膜中层厚度[IMT]),以确定颈总动脉内膜中层厚度(IMT)进展较快的受试者(IMT增加)>1.3毫米的颈总动脉。)

结果显示,68种蛋白质的变化预测了SCA的发展,其中PIgR2(聚合免疫球蛋白受体)、趋化因子配体18、CA1(碳酸酐酶1)、Fcγ受体IIa的水平较高,MMP10(基质金属肽酶10)、GT(胃泌素)、IL7R(白细胞介素7受体)的水平降低对SCA的发展最有预测力。与风险因素(年龄、性别、超重、高血压、低HDL-胆固醇、高甘油三酯)相比,这7种蛋白提高了SCA发病的敏感性和特异性,AUC=0.747([0.707-0.784];P<0.001)。

反之,有25种蛋白质预测颈总动脉IMT进展更快。其中,IL7D(白细胞介素7)、趋化因子配体1的增加,以及TNFS13B(TNF超家族成员13b)的减少,与相同的风险因素相比,显著提高了预测颈总动脉IMT加速发展的敏感性和特异性(AUC=0.719[0.680-0.756]; P<0.001)。

综上,这组新的循环蛋白可被视为临床前动脉粥样硬化发展的标志物。所确定的预测SCA与IMT进展的蛋白质的差异可能反映了不同的病理因素。
