JAMA Psychiatry:体力活动与抑郁风险之间的关联

抑郁症是现在最常见的一种心理疾病,以连续且长期的心情低落为主要的临床特征,是现代人心理疾病最重要的类型。

抑郁症是造成精神健康相关疾病负担的主要原因,也是全球致残的主要原因,2019 年约有 2.8 亿人受到影响。预防抑郁症需要有效的干预措施,体育锻炼可以预防抑郁症的发生。一项前瞻性研究的荟萃分析报告称,与体育活动水平低的人相比,体育活动水平较高的人患抑郁症的几率降低17%。但是大多数情况下,使用低活动与高活动的二元分类来实现对身体活动暴露的不同估计的协调,但这种方法会导致信息丢失,并且不能告诉我们一系列身体活动剂量的风险变化。因此,这项系统评价和荟萃分析的目的是调查体育活动与抑郁症之间的剂量反应关系。

研究人员通过搜索PubMed、SCOPUS、Web of Science、PsycINFO 等数据库,将检索日期截止为 2020年11月12日之前的相关文献都找了出来,纳入的研究包含 3 种或更多暴露水平的体力活动和 3000 名或更多成年人和 3 年或更长的随访时间的抑郁症风险估计。主要观察结果是抑郁症的发生率以及使用经过验证的抑郁筛查截止值确定的抑郁症状的危险因素。

 

本项荟萃分析共纳入15项研究,包括 191130 名参与者。研究人员观察到体力活动和抑郁之间的反向曲线剂量反应关联,而且在较低的活动量下具有更陡的关联梯度(P < .001)。相对于没有报告任何活动的成年人,那些累积了推荐体力活动量的一半的人抑郁风险降低了 18%(95% CI,13%-23%)。每周累积推荐量为 8.8 mMET 小时的成年人抑郁风险降低 25%(95% CI,18%-32%)。但是更多的身体活动量会减少额外的潜在益处和更大的不确定性。根据对纳入队列中暴露率的估计,如果活动较少的成年人达到了当前的体育活动建议,则可以预防 11.5%(95% CI,7.7%-15.4%)的抑郁症风险。

 

这项对体力活动与抑郁之间关联的系统评价和荟萃分析表明,体力活动对心理健康有显着的益处,即使是低于公共卫生建议的水平也有很好的预防作用。因此,临床医生应鼓励患者增加身体活动以改善心理健康。
