在众多癌基因中,RAS基因家族在人类肿瘤中突变概率最高,RAS突变肿瘤占人类所有恶性肿瘤的三分之一,其中KRAS作为RAS基因家族中的主要亚型,与肺癌、结肠癌胰腺癌有密切的关系。KRAS基因突变,主要集中在第12,13及61号密码子位置,其中第12号密码子位置的突变占到80%以上,其中KRAS G12C突变占所有KRAS突变的12%,其第12位的甘氨酸突变成半胱氨酸。临床研究显示,KRAS抑制剂Adagrasib和Sotorasib对KRAS G12C肿瘤具有良好的治疗效果,但其耐药性研究尚不充分。

近日研究人员采集了Adagrasib单一疗法治疗的 KRASG12C突变肿瘤患者肿瘤组织样本,进行基因组和组织学分析,通过细胞实验研究KRAS G12C 抑制剂耐药性突变

共有38名患者参与研究,其中27名非小细胞肺癌患者,10名结直肠癌患者,1名阑尾癌患者。在17名患者(队列的45%)中检测到疑似adagrasib耐药突变,其中7名患者(队列的18%)具有多重一致性。获得性KRAS改变包括:G12D/R/V/W、G13D、Q61H、R68S、H95D/Q/R、Y96C,以及KRASG12C 等位基因的高水平扩增。获得性旁路机制包括MET放大;NRAS、BRAF、MAP2K1和RET的激活突变;致癌融合,包括ALK、RET、BRAF、RAF1、FGFR3;NF1和PTEN功能缺失突变。在9例肺腺癌患者中,有2例观察到鳞状细胞癌组织学转变,未发现任何其他耐药机制。
