Circulation:运动员终身运动量与冠脉粥样硬化的关系

已知更高的身体活动水平与心血管事件的风险较低相关。然而关于运动和CVD结果的剂量-效应关系以及大量运动是否可能加速冠状动脉粥样硬化仍存在争议。这项研究旨在探讨终身运动量与冠状动脉粥样硬化的关系。

对从事竞技或娱乐休闲运动的中年男子进行非对比度和对比度增强的计算机断层扫描,以评估冠状动脉钙化(CAC)和斑块特征。参与者报告自身的终身运动史类型。运动量乘以任务的代谢当量(MET)的得分以计算MET-min /周。参与者被分类为<1000 MET-min /周,1000-2000 MET-min /周或> 2000 MET-min /周。

共纳入284名男性(55±7岁)。CAC存在于150名(53%)参与者中,中位CAC评分为35.8 [9.3-145.8]。终身运动量> 2000 MET-min /周(n=75)的运动员具有明显较高的CAC评分(9.4 [0-60.9] vs 0 [0-43.5],P=0.02)和CAC患病率(68%,调整OR=3.2(95%CI:1.6-6.6))和斑块(77%,调整OR = 3.3(95%CI:1.6-7.1)),<1000 MET-min /周(n=88,分别为43%和56%)。剧烈的运动强度(≥9METs)与CAC(调整OR=1.47(95%CI:1.14-1.91))和斑块(调整OR=1.56(95%CI:1.17-2.08))有关。在CAC> 0的参与者中,CAC评分(P= 0.20)、面积(P= 0.21),密度(P=0.25)和区域(P=0.20)在各运动量组间没有差异。在有斑块的参与者中,运动最多组(> 2000 MET-min /周)的混合斑块患病率较低(48%对69%,调整OR=0.35(95%CI:0.15-0.85),而且与运动最少组(<1000 MET-min /周)相比,最常见只有钙化斑块(38%对16%,调整OR = 3.57(95%CI:1.28-9.97))。
