每天吸入Molgramotim治疗可改善自身免疫性肺泡蛋白沉积症患者肺部功能

自身免疫性肺泡蛋白沉积症(aPAP)是一种罕见疾病,患者肺泡表面活性物质的合成、分泌异常和清除异常可导致肺泡蛋白沉积和低氧血症。粒细胞-巨噬细胞集落刺激因子(GM-CSF)信号通路在aPAP病理过程中发挥关键作用。近日研究人员考察了吸入GM-CSF Molgramostim对aPAP患者的动脉血氧分压的影响。
在一项双盲、安慰剂对照试验中, aPAP患者随机接受重组GM-CSF Molgramostim(300 μg,每天吸入一次,连续治疗组),每隔一周接受一次吸入治疗(间断治疗组)或安慰剂。研究的主要终点是24周时肺泡-动脉氧浓度差异(A-aDO2)与基线的变化。
 
138名患者参与研究,其中46名患者接受连续的Molgramotim治疗,45名患者接受间歇性Molgramotim治疗,47名患者接受安慰剂治疗。第24周时A-aDo2与基线水平的变化比较显示,连续Molgramostim组患者的改善程度高于安慰剂组(分别为-12.8 mm Hg vs -6.6 mm Hg;估计治疗差异为-6.2 mm Hg)。与安慰剂相比,接受连续Molgramostim治疗患者在次要终点方面也有更大的改善,包括第24周时圣乔治呼吸问卷总分变化(-12.4分vs -5.1分;估计治疗差异-7.4分)。对于多个其他终点,连续Molgramotim治疗组的改善程度大于间歇性Molgramotim治疗组。组间不良事件和严重不良事件发生率相似,但连续Molgramostim组胸痛患者的百分比更高。
