嗜酸性粒细胞增多与COVID-19患者结局改善之间的关联取决于iCS。需要未来的随机对照试验来确定iCS的作用及其与嗜酸性粒细胞增多症在COVID-19治疗中的相互作用。

除了它们的促炎作用外,嗜酸性粒细胞还具有抗病毒特性。同样,研究人员发现吸入皮质类固醇(iCS)可抑制冠状病毒的体外复制,并与2019冠状病毒病(COVID-19)患者预后改善有关。然而,两者之间的相互作用及其对COVID-19的影响需要进一步评估。近日,过敏性疾病领域权威杂志Journal of Allergy and Clinical Immunology-In Practice上发表了一篇研究文章,研究人员旨在明确预先存在的血液嗜酸性粒细胞绝对计数(AEC)、iCS和COVID-19相关结局之间的关联。

研究人员分析了来自克利夫兰诊所COVID-19研究登记处(2020年4月1日至2021年3月31日)的数据。在检测呈阳性的82096名患者中,有46397人在SARS-CoV-2检测日期之前进行了血液细胞分类计数。该研究的终点包括住院需求、入住重症监护病房(ICU)和住院死亡率。在倾向加权和调整后估计了嗜酸性粒细胞增多对患者结局的影响。

在最终分析中纳入的46397名患者中,19506名患者既往有嗜酸性粒细胞增多症(>0.15x103细胞/μL),5011名患者接受了iCS,9096名患者(19.6%)住院,2129名患者(4.6%)需要入住ICU,1402名患者(3.0%)在住院期间死亡。

在iCS治疗的患者中,调整后分析发现嗜酸性粒细胞增多与住院(OR[95%CI]:0.86[0.79-0.93])、入住ICU(OR[95%CI]:0.79[0.69-0.90])和死亡率(OR[95%CI]:0.80[0.68-0.95])之间存在相关性,但在未经治疗的患者中则没有。在未接受iCS治疗的患者中,AEC与估计的住院、ICU入院和死亡率之间的相关性呈非线性(U形),而在接受治疗的患者中呈负相关。

由此可见,嗜酸性粒细胞增多与COVID-19患者结局改善之间的关联取决于iCS。需要未来的随机对照试验来确定iCS的作用及其与嗜酸性粒细胞增多症在COVID-19治疗中的相互作用。
