JACC：多种新生物标志物可预测心衰伴保留型射血分数患者的预后

心衰伴保留型射血分数（HFpEF）尚需要更好的风险分层策略，本研究的目的是评估靶向血浆多标记方法在增强HFpEF表型特征和风险预测中的价值。

本研究对TOPCAT临床研究中的379名参与者进行了49个血浆标志物的检测，并评估了生物标志物与全因死亡率和心衰相关住院（DHFA）风险的相关性。结果发现了两个大的、紧密相关的生物标记物簇，包括纤维化/组织重塑、炎症、肾损伤/功能障碍和肝纤维化的生物标记物。其他生物标志簇由矿物代谢的神经激素调节因子、中间代谢和心肌损伤的生物标记物组成。多个生物标记物可预测DHFA的发生，包括2个与矿物代谢/钙化有关的生物标记物（成纤维细胞生长因子-23和OPG[骨保护素]）、3个炎症生物标记物（肿瘤坏死因子α、sTNFRI[可溶性肿瘤坏死因子受体I]和白细胞介素-6）、YKL-40（与肝损伤和炎症有关）、2与中间代谢和脂肪细胞生物学相关的生物标记物（脂肪酸结合蛋白-4和生长分化因子-15）、血管生成素-2（与血管生成相关）、基质金属蛋白酶-7（与细胞外基质转换相关）、ST-2和N末端前-B型利钠肽。

研究结果显示，多种新的病理生理领域的循环生物标志物可预测心衰伴保留型射血分数患者的预后。
