胆红素是脑卒中保护因素! CSPPT全新分析

近日,一项中国卒中一级预防试验(CSPPT)事后分析表明,高血压患者中,血清总胆红素或直接胆红素与首次缺血性脑卒中风险负相关。

近日,一项中国卒中一级预防试验(CSPPT)事后分析表明,高血压患者中,血清总胆红素或直接胆红素与首次缺血性脑卒中风险负相关。

研究显示,在高血压患者中,与处于第一分位血清胆红素水平(<9.3 μmol/L)者相比较,第三分位水平(12.9~34.1 μmol/L)者首次缺血性脑卒中风险降低25%。

研究还发现,直接胆红素三分位水平(2.5~24.8 μmol/ L)者,也比第一分位水平(<1.6 μmol/L)者首次缺血性脑卒中风险低23%。

不过,未发现血清总胆红素和直接胆红素水平与首次出血性脑卒中相关。

血清胆红素这种内源性抗氧化物质与机体总抗氧化能力呈正相关,之前有研究显示,其抗氧化活性与其形式无关。

还有研究发现,血清胆红素第与脂质和脂蛋白的氧化以及内皮损害有关,进而促成动脉粥样斑块和动脉血栓形成。

之前还有研究显示,2型糖尿病患者应用增加胆红素药物阿扎那韦后,抗氧化能力和内皮依赖性血管舒张能力改善。

研究者表示,如果该研究今后得到进一步证实,常规检测胆红素或有助于在高血压患者中确定缺血性脑卒中高危患者。

这项研究共纳入CSPPT试验中19906例高血压患者,中位随访4.5年期间,608人发生脑卒中,其中包括489例缺血性脑卒中,117例出血性脑卒中,和2例不明确类型脑卒中。
