两种血液因子帮助阿尔兹海默症的个体化预后治疗

该研究共纳入了来自瑞典BioFINDER研究和阿尔茨海默病神经影像学倡议(ADNI)的573名MCI患者。主要结果是纵向认知和转化为阿尔茨海默病(AD)痴呆的情况。

全球约有5000万痴呆症患者,预计到2030年患病率将翻倍。50%至70%的痴呆症病例是由阿尔茨海默病(AD)引起的。痴呆症阶段前有轻度认知障碍(MCI)。准确的预后在MCI中是很重要的,因为它可能会导致认知能力下降和痴呆(由于AD或其他疾病),或者是良性和稳定的后果。如果AD的疾病改良治疗变得可用,准确的预后可能对指导MCI患者的治疗很重要。

最近,研究人员在Nature Aging杂志发文,报导其开发了使用β-淀粉样蛋白(Aβ)、tau和神经变性的血浆生物标志物对轻度认知障碍(MCI)的认知能力下降进行个体化风险预测的模型。

该研究共纳入了来自瑞典BioFINDER研究和阿尔茨海默病神经影像学倡议(ADNI)的573名MCI患者。主要结果是纵向认知和转化为阿尔茨海默病(AD)痴呆的情况。

结合tau在苏氨酸181的磷酸化(P-tau181)和神经丝光(NfL),但不包括Aβ42/Aβ40的模型,在所有模型中预测表现最好(BioFINDER中4年转化为AD的曲线下面积=0.88,在ADNI中得到验证),比年龄、性别、教育和基线认知的基本模型更强,表现与脑脊液生物标志物相似。

因此,该研究介绍了一种基于组合血浆生物标志物模型的MCI个体化预后的公开在线工具。组合血浆生物标志物可能对识别MCI个体在临床试验和临床实践中会进展到AD痴呆具有很高的价值。
