J Clin Oncol:吉非替尼辅助治疗EGFR突变型NSCLC的5年预后

ADJUVANT-CTONG1104是一项随机化III期试验,显示吉非替尼辅助治疗,相比长春瑞滨联合顺铂(VP),可显著提高EGFR突变的手术切除的II-IIIA(N1-N2)期非小细胞肺癌(NSCLC)患者的无病存活期(DFS)。现本文报道了该研究的最终总体存活(OS)情况结果。

该试验在2011年9月-2014年4月期间从27个地点招募了222位受试患者,1:1随机分至两组(各111位患者)。吉非替尼组:辅助治疗24个月,VP组:3周一疗程,连用4个疗程。主要终点是DFS。次要终点包括OS、3年/5年DFS率和5年OS率。

两组的预后情况

中位随访80个月。吉非替尼组和VP组的中位OS分别为75.5个月和62.8个月(风险比[HR] 0.92,95% CI 0.62-1.36,p=0.674),5年OS率分别为53.2%和51.2%(p=0.784)

两组的后续治疗情况

68.4%的接受吉非替尼治疗的患者和73.6%的接受VP治疗的患者在进展期有接受后续治疗。与无后续治疗相比,后续靶向治疗可明显影响OS((HR 0.23,95% CI 0.14-0.38)

两组最新的DFS率

吉非替尼组和VP组的最新3年DFS率分别为39.6%和32.5%,5年DFS率分别为22.6%和23.2%

总之,吉非替尼辅助治疗用于携带EGFR突变的早期NSCLC患者相比标准化疗可提高患者的DFS。虽然这种DFS优势并未转化为明显的OS差异,但与历史数据相比,吉非替尼辅助治疗的OS是该类患者中观察到的最长的OS之一。
