JAHA:心肺复苏过程中吸入纯氧会导致大脑氧化损伤

心肺复苏(CPR)过程中的高氧可能导致线粒体衍生活性氧的氧化损伤,本研究假设在心肺复苏过程中暴露于纯氧会导致大脑高氧、线粒体衍生活性氧种类增加、氧化损伤增加,与暴露于21%氧的患者相比,存活率相似。

心肺复苏(CPR)过程中的高氧可能导致线粒体衍生活性氧的氧化损伤,本研究假设在心肺复苏过程中暴露于纯氧会导致大脑高氧、线粒体衍生活性氧种类增加、氧化损伤增加,与暴露于21%氧的患者相比,存活率相似。

本研究将4周龄仔猪(n=25)进行窒息性心脏骤停,在恢复自然循环后10分钟内,随机分组,分成0.21(n=10)或1.0吸入氧(n=10)的心肺复苏,假手术组有5只。分析结果发现纯氧组和0.21组在全身血流动力学、脑血流及生存率方面无明显差异(8/10)。纯氧组在心肺复苏和自然循环恢复后大脑相对高氧,且脑线粒体活性氧产生量高于0.21组(p<0.03)。另外,与0.21组(P<0.05)和假手术组(P<0.001)相比,暴露于纯氧气会导致蛋白质和脂类的脑氧化损伤增加,表现为蛋白质羰基和4-羟基酮显着升高。

研究结果显示,心肺复苏过程中吸入纯氧会引起大脑高氧,导致线粒体来源的活性氧增加和氧化损伤。
