Crit Care:肾上腺素对心肺复苏期间脑血管和全身血流动力学的影响

尽管存在争议,肾上腺素仍然是心肺复苏(CPR)的主要手段。最近的动物研究表明,肾上腺素可能会降低脑血流量(CBF)和脑氧合,增加CPR期间的神经系统损伤。近日,危重病医学领域权威杂志Critical Care上发表了一篇研究文章,研究人员调查了猪模型静脉注射肾上腺素对脑血管的影响。这项研究的主要目的是确定(1)肾上腺素剂量在CPR期间是否对CBF和脑组织氧合具有显著的急性效应,以及(2)每剂肾上腺素的效应是否与第一次显著不同。

一个月大的仔猪(n=20)窒息7 min,心室颤动,CPR时间10-20 min。在CPR的2、6、10、14和18分钟注射肾上腺素(20 mcg/kg)。同时记录脑血流和脑组织氧合的有创(激光多普勒、脑组织氧张力[PbtO2])和无创(扩散相关光谱法和漫射光谱法)指标。将随后注射的肾上腺素效果与第一次注射的效果进行比较。

在CPR期间首次注射肾上腺素时,通过有创和无创测量,CBF和脑组织氧合增加>10%(p<0.001)。肾上腺素对CBF和脑组织氧合作用的影响随剂量的增加而降低。通过第五次肾上腺素剂量,脑组织氧合的CBF没有明显增加。在窒息(斜率效应为1.3,p<0.001)和CPR(斜率效应为0.20,p<0.001)期间,有创和无创CBF测量值高度相关。
