Circ Res:巨噬细胞膜受体Lgr4参与急性心肌梗死后心脏损伤修复机制

急性心肌梗死是冠脉血液循环中断引起的急性、持续性心肌缺血和缺氧导致的心肌坏死。仍有患者因过量的心肌坏死或梗死区域修复障碍而发生恶性心脏重构,导致心力衰竭甚至心脏破裂,是制约急性心肌梗死预后的重要因素。

急性心肌梗死是冠脉血液循环中断引起的急性、持续性心肌缺血和缺氧导致的心肌坏死。目前,我国急性心肌梗死发病率的“拐点”仍未到来。虽然通过介入治疗或搭桥手术可以开通阻塞的冠脉,但是仍有一些患者因过量的心肌坏死或梗死区域修复障碍而发生恶性心脏重构,导致心力衰竭甚至心脏破裂,是当前制约急性心肌梗死预后的重要因素。

免疫炎症反应在心肌梗死后损伤修复中的作用是近年来的研究热点。巨噬细胞作为一类固有免疫细胞,在心肌梗死后的炎症反应中具有重要的作用。巨噬细胞在心肌梗死后心肌组织中的浸润、活化是一个极其复杂的过程,巨噬细胞通过炎症表型转化调控心肌梗死后炎症反应和抗炎反应的平衡,在心肌梗死炎症反应阶段和修复时期均发挥着重要的调控作用。巨噬细胞适当及时地由促炎表型转换为修复表型可以限制炎症,有助于心肌梗死后的修复,进而改善心肌梗死后心脏重构和预后。因此,明确巨噬细胞功能表型的内源性调控机制对寻找心肌梗死早期干预靶点具有重要意义。

近日,上海交通大学医学院附属瑞金医院张瑞岩、闫小响和王计秋研究团队在Circulation Research杂志上发表了题为 Lgr4 governs a pro-inflammatory program in macrophages to antagonize post-infarction cardiac repair 的研究论文(黄春恺博士为该论文的第一作者)。该研究首次发现富含亮氨酸重复序列的G蛋白偶联受体(leucine-rich repeat-containing G protein-coupled receptor,LGR)家族成员Lgr4调控巨噬细胞促炎激活参与心肌梗死后损伤修复的关键作用和机制。

Lgr4作为膜受体,介导胞膜内外信号转导,除了激活经典的cAMP/PKA/CREB(cAMP responsive element binding protein)信号通路,还可与内源性配体R-spondins(RSPOs)家族蛋白结合,增强Wnt信号通路的激活。Lgr4基因调控细胞的增殖和分化,在诸多组织器官中均有着极其重要的作用。该团队既往系列研究发现Lgr4参与水盐代谢、肝脏脂质代谢、白色脂肪组织棕色化和肥胖、以及破骨细胞分化和骨质疏松等疾病的发生发展,表明Lgr4基因的功能重要。

基于前期基础,在本项研究中,结合小鼠心肌梗死模型和急性心肌梗死患者心肌组织标本,作者进一步发现在急性心肌梗死的早期Lgr4高表达于梗死灶浸润的活化状态巨噬细胞。作者利用Cre/loxP体系构建巨噬细胞特异性Lgr4基因敲除小鼠。功能研究发现,巨噬细胞特异性Lgr4敲除小鼠心肌梗死后出现心功能和心脏重构明显改善、梗死面积轻度减小、急性期心脏破裂发生率降低,远期生存率提高等表型。巨噬细胞特异性Lgr4敲除减少心肌梗死灶白细胞和促炎性Ly6Chigh单核/巨噬细胞的浸润,从而减轻梗死心肌组织局部炎症反应和梗死边缘区细胞凋亡,并促进心肌梗死区域修复,有助于梗死灶I型胶原沉积、肌成纤维细胞增生、新生血管生成和富含胶原的瘢痕组织形成。

转录组学数据也证实众多介导促炎反应的基因和信号通路在巨噬细胞Lgr4敲除小鼠的梗死心肌组织中受到抑制,进一步支持巨噬细胞特异性Lgr4敲除小鼠心肌梗死预后改善的表型。为了研究其具体调控机制,作者发现Lgr4的内源性配体RSPOs家族成员在梗死心肌组织表达未上调且呈低丰度表达,此外,Wnt信号通路在巨噬细胞特异性Lgr4敲除的梗死心脏中未受到显着影响,提示RSPOs-Lgr4-Wnt信号轴有可能不参与对梗死区域浸润的巨噬细胞功能表型的调控。基于转录组学数据,作者发现转录因子AP-1(activator protein-1)是Lgr4调控心肌梗死灶巨噬细胞促炎激活的下游靶点。AP-1是由Fos和Jun家族成员组成的转录因子复合物,作为一种经典的促炎转录因子,介导巨噬细胞的促炎激活,而AP-1的激活与Fos(c-Fos、Fosb、Fosl1、Fosl2)和Jun(c-jun、Junb、Jund)基因家族成员诱导性转录相关。作者通过体外实验进一步证实,Lgr4在巨噬细胞促炎激活过程中表达增高,通过经典G蛋白偶联受体信号通路,增强cAMP应答元件结合蛋白(cAMP response element-binding protein,CREB)介导的Fos基因家族成员(c-Fos、Fosb、Fosl1)转录激活,协同增强AP-1活性,从而启动下游促炎基因表达,促进巨噬细胞的促炎性激活。

该研究表明Lgr4在巨噬细胞促炎激活和心肌梗死后重构过程中具有关键作用,进一步丰富了心血管炎症免疫理论,也为急性心肌梗死的抗炎治疗提供了新的早期干预靶点。
