        慢性功能性便秘(CFC)是一种常见的肠道功能性疾病,其特征是排便次数减少和大便干燥。它还可能伴有肛门肿胀和排便。尽管有很多用于便秘的药物,但对于某些患者来说效果欠佳,肠道菌群与肠道功能密切相关,宿主微生物组的紊乱已被证明会导致肠道功能障碍。尚不清楚微生物是否参与CFC的发病机制。但是,最近的研究表明,便秘患者的肠道菌群失调。CFC患者的肠道菌群数量和成分均异常。CFC患者的肠道菌群多样性发生了变化,这主要是由于双歧杆菌和乳杆菌的减少以及拟杆菌,梭菌和肠杆菌的显着增加。便秘所涉及的肠道菌群的发病机理尚不清楚。然而,肠内细菌代谢产物,如内毒素和短链脂肪酸,与肠平滑肌收缩和肠蠕动功能密切相关。短链脂肪酸可促进5-羟色胺(5-HT)的分泌,并加速结肠传输的速度。此外,内毒素是革兰氏阴性细菌细胞裂解的一种成分,可促进一氧化氮(NO)的产生。NO扩散到平滑肌细胞中,并激活可溶性尿苷环化酶,引起环鸟苷单磷酸(CGMP)升高,降低细胞内Ca 2+浓度并引起平滑肌松弛。因此,本项研究旨在评估粪便微生物菌群移植(FMT)对于常规便秘药物无效治疗的慢性功能性便秘(CFC)的安全性和有效性。

 

        研究人员对34例CFC患者进行了FMT治疗(三轮,通过胃镜进行)。临床量表,包括韦克斯纳便秘评分作为主要疗效指标评价工具。每次治疗后以及随访2个月和3个月时分别进行评分。次要评估指标包括便秘症状的自我评估,患者评估便秘生活质量的问卷,布里斯托尔粪便形式量表以及宗格的抑郁和焦虑量表。治疗前后评估胃肠蠕动,胃动素,胃泌素,一氧化氮(NO)和5-羟色胺(5-HT)水平差异。通过16S核糖体核糖核酸(rRNA)测序评估肠道菌群变化。

 

        研究结果显示粪便菌群移植的临床治愈率为73.5%(25/34),临床缓解率为14.7%(5/34),无效率为11.8%(4/34)。临床量表数据表明FMT治疗有效。此外,FMT治疗可促进肠蠕动,增加肠胃蠕动以及增加血清NO和5-HT水平。16S rRNA测序数据表明,高含量的拟杆菌,克雷伯氏菌,大摩纳哥,丹毒和菊科可能是便秘的原因,而高水平的普雷沃特氏菌,酸氨基球菌和丁酸杆菌可能是治愈便秘的主要因素。
