北京时间8月1日,《风湿病年鉴》(Annals of the Rheumatic Diseases)(IF=16.102)在线发表了题为“Systemic evaluation of the relationship between psoriasis,psoriatic arthritis and osteoporosis: observational and mendelian randomization study”的研究文章,阐明了银屑病(俗称牛皮癣)及银屑病性关节炎与骨质疏松间的关系。

西湖大学生命科学学院PI郑厚峰为此文通讯作者,2018级博士研究生夏江威为第一作者。西湖大学为此项研究工作的独立通讯单位。参与单位包括密西根大学,浙二医院,滨州医学院和江西医学高等专科学校等。西湖大学超算中心亦为项目提供技术支持。

为什么做皮肤病与骨质疏松关系的研究呢?

郑厚峰曾经是一位皮肤科医生,在医学博士期间的研究课题就是围绕银屑病的遗传学研究,在加拿大McGill University做博士后期间,郑厚峰转为做骨代谢相关性状的遗传学研究,所以对两个疾病都比较了解。

银屑病是一种常见的自身免疫性皮肤病,在欧洲人群的发病率是2.5%,亚洲人群发病率是0.1%-0.5%,其中最严重的并发症是银屑病性关节炎。尽管以前也有很多研究关注银屑病跟骨质疏松间的关系,但是由于受到潜在的混杂因素影响,或者无法完整收集到足够多的数据,很多研究的结果不一致,导致目前对银屑病是否能够导致骨质疏松依旧存在着争议。

2017年《英国皮肤病学杂志》(British Journal of Dermatology)发表评论文章“Psoriasis andosteoporosis: the debate continues”。如何确定银屑病跟骨质疏松间是否存在归因关系吸引越来越多研究者的注意,也成为临床上的一个重要问题。所以郑厚峰团队在大数据的基础上,更加全面地分析了两者的相互关系,并且得出了可靠的结论,为临床指南提供了新证据。

他们研究发现,银屑病性关节炎(Psoriatic arthritis)患者与对照组相比,骨量明显减少,但这个关联关系并不由遗传因素决定,也就是不具有归因关系。他们推测可能是继发导致骨质疏松(例如通过服用药物或者缺乏锻炼)。

为了证明这一点,他们纳入银屑病性关节炎患者服用药物情况和体育锻炼的因素,通过中介分析(Mediation analysis)方法,发现银屑病性关节炎对于降低骨密度的风险效应果然有63%是通过服用药物(甲氨蝶呤,环孢素)间接介导的,而体育锻炼的中介作用为8%。基于非归因假设,非关节炎型银屑病患者因为没有继发效应,并不是骨质减少或骨质疏松潜在的风险因素。

此外,他们在研究中还发现,男性银屑病性关节炎患者与对照组相比,低骨量特征的显著性高于女性。他们推测可能的原因是纳入研究的女性大部分为绝经后的女性,缺乏雌激素可能相比银屑病性关节炎对骨密度的影响更大。

简单总结郑厚峰团队的研究成果就是:

首先,银屑病(包括关节炎型)与骨质疏松之间没有归因关系(Causal relationship)。

但是,银屑病性关节炎作为一种重型的银屑病,它的临床治疗会适用甲氨蝶呤或环孢素等可能影响骨重建的药物,从而导致骨质疏松的发生。

以前的观察性研究结果相互不能验证,有可能是因为:(1)纳入的银屑病患者没有分疾病亚型,比如银屑病性关节炎患者没有单独分析,(2)纳入的疾病混杂因素没有包括药物治疗情况。

需要强调的是,此研究就纳入英国大型样本数据库(UK Biobank,https://www.ukbiobank.ac.uk/)的432,513例样本数据进行横断面观察性研究,在这个样本量下,有足够大的统计学把握度(Statistical power)来研究暴露(Exposure)和结局(Outcome)的关系。

对于临床应用而言,郑厚峰团队建议银屑病性关节炎患者(特别是接受甲氨蝶呤或环孢素治疗的患者)应定期筛查骨量降低和骨质疏松的风险,并应提供适当的健康管理,以降低骨折风险。
