Neurology:痴呆前的MCI可防可治,近半数可能会回归正常!

轻度认知损害(MCI)是一种介于正常认知与痴呆的中间临床状态。

轻度认知损害(MCI)是一种介于正常认知与痴呆的中间临床状态。虽然正常衰老过程中常观察到特定的细微认知改变,但越来越多的证据表明,某些形式的认知损害可视为神经变性疾病的早期表现,最终将导致痴呆。

MCI作为一种综合征,是指不满足痴呆标准但程度超过正常衰老的认知损害。最初,人们认为MCI主要是记忆损害并且是阿尔茨海默病(AD)的前驱状态。后来,人们认识到MCI在临床表现、病因、预后和患病率方面的多样化,将MCI的范围扩大到了其他认知域,从而扩展了在其他痴呆的前驱阶段进行的综合征早期检测。

为了研究发生MCI的社会人口学和医学预测因素以及后续的MCI过程,包括持续的MCI诊断、认知正常的分类和进展为痴呆的风险,来自美国纽约哥伦比亚大学脑病中心的专家开展了相关研究,结果发表在Neurology杂志上。

研究人员在一个以社区为基础的队列中,使用标准的指南对MCI进行诊断。痴呆症的诊断是基于临床共识。Cox回归估计了与几个预测因素相关的MCI事件的危险比。修正的泊松回归估计了与发病后随访时诊断状态的预测因素相关的相对风险。

结果显示,在基线的2903名认知正常的参与者中,752人在平均6.3(SD=4.5)年的时间里发生了MCI(发病率:56/1000人年)。APOE ε4的存在和较高的医疗负担增加了发生MCI的风险,而更多的教育年限、更多的休闲活动和更高的收入减少了这种风险。

从认知正常到轻度认知障碍最后痴呆的过程

在发病的MCI病例中,经过平均2.4年的随访,12.9%的人发展为痴呆,9.6%的人功能下降, 29.6%的人继续符合MCI标准,47.9%的人不再符合MCI标准。多领域的MCI、APOE ε4的存在、抑郁症状和抗抑郁药的使用增加了进展为痴呆的风险。

这项以社区为基础的研究表明,几乎有一半被诊断为MCI的人在随访时被归类为认知正常。事发MCI的预测因素与随后的MCI过程明显不同;这些发现可以完善对那些出现MCI的人的认知和功能过程的预期。
