J Leukoc Biol：警惕，牙周炎可能会影响全身炎症

炎症是身体对感染或损伤的反应，旨在促进组织和整体的平衡，是常见非传染性疾病的一个关键驱动因素。牙周炎是一种常见的慢性炎症性疾病，由特定的口腔菌群失调引起，其特点是保持牙齿的软硬组织逐渐丧失，会引发了机体一系列的体液炎症反应，但对其对循环炎症细胞谱的影响知之甚少。近日，发表于J Leukoc Biol的一项研究系统评估了所有将牙周炎及其治疗与循环炎症细胞谱关联的证据。

研究筛查了从人员检索了6个数据库中，共有157项研究符合定性综合的条件，其中29项研究符合meta分析的条件。荟萃分析结果显示，牙周炎患者的循环CD4⁺、CD4⁺CD45RO⁺、IFNγ-表达的CD4⁺和CD8⁺T细胞、CD19⁺CD27⁺和CD5⁺B细胞、CD14⁺CD16⁺单核细胞和CD16⁺中性粒细胞的平均数明显增加，但CD8⁺T和CD14⁺⁺CD16^-单核细胞的数量减少。定性综述显示，与健康对照组相比，牙周炎患者的外周血中性粒细胞始终显示出活性氧（ROS）的产生水平升高。一些证据表明，治疗牙周炎可以逆转ROS水平的升高，但在几个循环炎症细胞分析中发现的数据有限且不确定。

综上所述，该研究结果表明，牙周炎及其治疗与循环炎症细胞谱的改变有关。这些变化可能是解释牙周炎与其他合并症（如心血管疾病、糖尿病和类风湿性关节炎）的关键机制。进一步的研究应该集中在炎症模型上，以揭示这些关联的确切机制，并通过大型随机临床试验证明治疗牙周炎比治疗其他常见合并症患者的系统性并发症更有潜在好处。
