JAMA Netw Open:肥胖者运动可保护膝关节!美国10年随访研究!

美国西北大学学者10年随访研究表明,长期中等强度剧烈运动并不会增加高危人群膝关节炎罹患风险,反而可能有保护作用。

在这项研究中,有49.7%的人8年未进行过剧烈运动,42.5%的人经常久坐不动。

研究发现,长期中低强度运动可降低31%的10年X线诊断的膝关节炎风险,剧烈运动可降低25%的风险。

那些缺乏运动的人,年龄更大,更胖,膝关节疼痛更多见,大学教育程度比例更低,肌肉无力及抑郁更常见。

研究也发现,久坐与关节炎无关。

研究者指出,膝关节炎高危患者可安全进行中等强度运动,医务人员可考虑在早期将体育锻炼作为膝关节炎高危人群干预的一部分。

近日发表的国内首部《膝骨关节炎运动治疗临床实践指南》也指出,无论患者年龄和疾病严重程度,运动治疗应作为膝骨关节炎的一线治疗方法。

指南指出,运动治疗可减轻疼痛,改善躯体功能,并降低跌倒风险。只是偶尔带来有临床意义的不良事件。

对于膝关节活动度受限者,关节活动度锻炼可作为有氧运动和肌力锻炼的有效补充。

指南,推荐患者每周定期锻炼2~3次,逐渐养成规律运动的习惯。

美国这项研究共纳入1194例膝关节高危人群,相对有轻微症状但膝关节功能较好。其中女性58.4%,平均年龄58.4岁,平均体重指数26.8 kg/m2。其中的运动包括慢跑、游泳。骑自行车、打网球、跳舞、滑雪等。随访10年期间,13%的人经X线检查确诊得了膝关节炎。
