随着时间的推移，对进行血运重建术的有症状的PAD患者，针对不良事件的药物预防使用率显著增加，伴随着所有不良预后的发生风险降低

外周动脉疾病(PAD)患者心血管疾病发病率和死亡率的风险增加。采用抗血栓和他汀类药物进行药物预防是预防不良预后的主要治疗手段；然而，PAD患者的治疗往往不充足。

本研究概述了一组有症状的PAD患者血运重建术后药物预防和不良预后的时间变化。

纳入了2000年-2016年在丹麦进行下肢或腹部首次开放性手术或血管内血运重建术的所有患者。在调整了手术类型、治疗适应症、年龄、性别和心血管危险因素后，评估了阿司匹林、氯吡格雷和他汀类药物使用的时间变化和不良临床预后的1年原因特异性危险比。

2000年至2016年有症状的外周动脉疾病的标准化血运重建率

2000年-2016年期间，共32911位患者因症状性PAD进行了血运重建。平均年龄为69岁，且随时间的推移而增加，共发病负担也随之增加。

血运重建术后180天二级预防使用的时间变化

2000年-2004年和2013年-2016年的累计用药率均呈上升趋势，阿司匹林、氯吡格雷和他汀类药物的使用率分别从57.3%增加至了64.3%、从3.6%增加至了24.8%和从36.2%增加至了77.1%。与此同时，一年不良预后率降低。

不良预后的一年累计发生率

与2000年-2004年相比，2013年-2016年主要不良心血管事件的校正风险比为0.73（95% CI 0.62-0.84）、主要不良肢体事件的校正风险比为0.92（0.85-1.00）、心肌梗死的为0.6（0.48-0.74）、缺血性卒中的为0.94（0.39-0.76）、大出血的为0.92（0.75-1.12）、心血管死亡的为0.54（0.39-0.76）和全因死亡率的0.80（0.72-0.88）。

与2000年-2004年相比，2013年-2016年MACE和MALE的校正风险比

这些预后改善在2000年-2004年至2005年-2008年期间最为显著。与此相反，2013年-2016年与2000年-2004年相比的重大截肢的校正风险比为1.00（0.90-1.11）。

总之，随着时间的推移，对进行血运重建术的有症状的PAD患者，针对不良事件的药物预防使用率显著增加，伴随着所有不良预后的发生风险降低，除了大截肢。
