肌萎缩侧索硬化症(ALS)是一种运动神经系统进行性退行性疾病。通过经颅磁刺激和阈值追踪神经传导研究证明了ALS患者皮层和脊髓运动神经元的兴奋性较高,尽管兴奋性指标尚未作为临床试验药效学的生物标志物。Ezogabine是一种癫痫病成年患者局部发作辅助治疗药物,近日研究人员考察了Ezogabine对ALS患者皮质和脊髓运动神经元兴奋性的影响。
 
本次研究为双盲、安慰剂对照II期随机临床试验,在美国东北部12个ALS中心进行。参与者被随机接受高剂量(900mg/天)或低剂量(600mg/天)的Ezogabine或安慰剂,为期10周。并在筛选、基线检查、第6周和第8周完成临床评估和神经生理学测量。研究的主要结果是短时间皮质内抑制变化(SICI;SICI-1以反映抑制作用),次要结果包括经颅磁刺激测量的皮层运动神经元兴奋性水平(包括静息运动阈值)和阈值跟踪神经传导研究测量的脊髓运动神经元兴奋性(包括强度-时间常数)。
 
65名ALS患者参与研究,其中安慰剂组23人,600 mg/天Eezogabine组23人,900 mg/天Ezogabine组19人,参与者中男性45名(69.2%),平均年龄为58.3岁。与安慰剂组相比,900 mg/天Ezogabine组患者的SICI-1增加了53%(平均比率1.53),而600 mg/天组SICI-1没有变化(平均比率1.15)。600 mg/天Ezogabine组与安慰剂组相比,静息运动阈值增加(平均比率4.61),但900 mg/天Ezogabine组无显著变化(平均比率1.95)。强度-期间时间常数结果显示,900 mg/天Ezogabine组导致兴奋性降低呈现剂量依赖性。
 
研究发现,Ezogabine降低了ALS患者的皮层和脊髓运动神经元的兴奋性,提示这种神经生理学指标可以作为临床试验的药效学生物标志物。
