质子MR技术为研究与多发性硬化症发病机制潜在相关的几种代谢物之间的关系提供了可能性。多发性硬化症中最报道的异常是N-乙酰天门冬氨酸(NAA)的减少和肌醇(MI)的升高。由于NAA几乎只存在于神经元中,这种代谢物水平的降低被解释为轴突和神经元完整性受损的证据,也被解释为MS中可逆线粒体功能障碍的标志。最近,在7.0T自由诱导衰减(FID)磁共振光谱成像(MRSI)方面取得进展,减少了部分体积误差,可在大约6分钟的扫描时间内更准确地描述WML的代谢变化。

近日,发表在Radiology杂志的一项研究评估7.0T FID MRSI对多发性硬化症患者的NAWM和CGM的病理表现进行描述和可视化的能力,并研究了其肢体障碍之间的关系,进一步明确多发性硬化疾病进展的病理生理

本研究于2016年1月至2017年12月期间,前瞻性地招募了患有多发性硬化症的参与者和年龄性别匹配的健康对照组。在白质病变、NAWM和CGM区域获得代谢率。使用协方差分析对基于扩展的残疾状态量表(EDSS)评分的MS相关残疾进行了亚组分析。

本研究共纳入了65名多发性硬化症患者(平均年龄±标准差,34岁±9岁;34名女性)和20名年龄和性别匹配的健康对照者(平均年龄,32岁±7岁;11名女性)。在多发性硬化症患者的NAWM的代谢图像上可以看到肌醇(mI)的高信号强度,而N-乙酰天门冬氨酸(NAA)的信号强度并没有降低。在所有MS亚组的半卵圆中心的NAWM中观察到较高的mI与总肌酸(tCr)的比率,包括无残疾的参与者(边缘平均值±标准误差,健康对照组。0.78±0.04;EDS 0-1:0.86±0.03[P=0.02];EDS 1.5-3:0.95±0.04[P<0.001];EDS≥3.5:0.94±0.04[P=0.001])。在MS残疾亚组中发现NAA与tCr的比率较低,NAWM(边缘平均值±标准误差,健康对照组。1.46 ± 0.04; EDSS 1.5-3: 1.33 ± 0.03 [P = .03]; EDSS ≥3.5: 1.30 ± 0.04 [P = .01])和CGM(边平均值±标准误差,健康对照: 1.42 ± 0.05; EDSS ≥3.5: 1.23 ± 0.05 [P = .006])也是如此。mI/NAA与EDSS相关(半卵圆中心的NAWM:r=0.47,P<0.001;顶叶NAWM:r=0.43,P=0.002;额叶NAWM:r=0.34,P=0.01;额叶CGM:r=0.37,P=0.004)。

 

 肌醇与N-乙酰天门冬氨酸(mI/NAA)比率的代谢图清楚地描述了在T1w/FLAIR成像中显示不明显的皮质下或皮质旁小病变(圆圈)(在三个多发性硬化症参与者中用箭头表示)。

综上所述,本研究表明,多体素7.0T自由诱导衰减磁共振波谱成像可以多发性硬化症颅内病变进行病理表现的显示及评估将这些以前看不见的变化考虑在内,可以加强对疾病活动的测量及评估,从而改善对疾病进展和当前治疗效果的监测。
