目前，关于红细胞分布宽度(RDW)在危重症中的预后价值仍有争议。本研究的目的是探讨入院时RDW对危重症成人住院和4年死亡率的预后价值。

目前，关于红细胞分布宽度(RDW)在危重症中的预后价值仍有争议。本研究的目的是探讨入院时RDW对危重症成人住院和4年死亡率的预后价值。

这是一项来自重症监护III(模拟III)医学信息集市数据库(1.4版)的回顾性队列研究。包括首次入住ICU的患者。使用结构化查询语言(SQL)提取患者入院时的RDW和严重程度评分。将患者分为训练集和验证集。采用ROC曲线、Kaplan-Meier曲线、Cox模型、净重分类指数(net reclassification index, NRI)、综合判别指数(integrated index, IDI)和nomogram分析RDW与住院及4年全因死亡率的关系。

本研究共纳入36,532例患者(21,090例为训练组，15,442例为验证组)。RDW升高与住院和4年死亡率升高显著相关。RDW对4年死亡率的预后价值与传统的严重程度评分无关。以传统的严重程度评分作为协变量，RDW的持续性NRI和IDI分别约为0.3-0.5和0.01-0.03。4年死亡率的NRI约为0.2-0.3,IDIs约为0.03-0.08。

研究结果表明，入院RDW可预测重症监护室成人重症患者的住院和4年死亡率，并可提供超过常规临床严重程度评分的额外预后价值。
