尽管心肌炎病例很少，但COVID-19 mRNA疫苗接种的益处-风险评估对于所有年龄段和性别组均倾向于有利的方面；因此，建议12岁及以上的人员都接种COVID-19疫苗。

心肌炎已被认为是COVID-19 mRNA疫苗接种后罕见的并发症，尤其是在年轻成人和青春期男性中。

近日，心血管领域权威杂志Circulation上发表了一篇研究文章，根据美国疾病控制中心(CDC)的数据，在12-39岁的人群中，接种每百万剂mRNA疫苗，心肌炎/心包炎发病率约为12.6例。

在报告的病例中，心肌炎患者表现为胸痛，通常在第二剂mRNA疫苗接种后2-3天内出现，并且伴有心肌肌钙蛋白水平升高。大多数心电图异常伴ST段抬高，所有受试患者的心脏MRI提示为心肌炎，且没有证据表明急性COVID-19或其他病毒感染。

该研究中有一例患者心肌病基因型为阴性，但针对某些自身抗原的自身抗体水平升高，以及自然杀伤细胞数量增加。虽然心肌炎的发生机制尚不清楚，但推测与严重急性呼吸综合征冠状病毒2(SARS-CoV-2)的刺突蛋白与自身抗原之间的分子模拟，触发某些患者既往存在的且失调的免疫途径，对mRNA的免疫反应和免疫途径的激活，以及失调的细胞因子表达相关。心肌炎病例男性占主导的原因尚未明确，但可能的解释是与免疫反应和心肌炎中性激素的差异以及对女性心肌炎的诊断不足有关。几乎所有患者的症状和体征都得到缓解，诊断标志物和影像学无论有无治疗都有所改善。

该研数据表明尽管心肌炎病例很少，但COVID-19 mRNA疫苗接种的益处-风险评估对于所有年龄段和性别组均倾向于有利的方面；因此，建议12岁及以上的人员都接种COVID-19疫苗。
