低级别浆液性卵巢癌：Binimetinib疗效有待进一步验证

美国亚利桑那州大学医学院Monk等报告的一项随机研究（MILO/ENGOT-ov11）显示，尽管低级浆液性卵巢癌（LGSOC）研究中的 MEK 抑制剂Binimetinib在疗效终点评估中显示抗瘤活性，KRAS突变可预测对Binimetinib的反应。（J Clin Oncol. doi：10.1200/JCO.20.01164）
 
在过去，LGSOC对化疗的反应较低。影响MAPK途径改变最常见的是KRAS/BRAF， 而 它们又占LGSOC的30％~60％。这项研究的目的是在LGSOC中评估一种有效的MEK1/2 抑制剂Binimetinib，它在多种肿瘤中均有活性。
 
患者按2︰1比例随机分组，分别接受Binimetinib（45 mg bid）与医生选择的化疗（PCC）。入组条件：≥1种含铂化疗，但化疗不超过3线后有可测量的LGSOC复发。主要终点是BICR评估的无进展生存期（PFS）。在期中分析时（2016年1月20日），共有 303 例患者随机。BICR评价的Binimetinib组的中位PFS为9.1个月，PCC组的中位PFS为10.6个月（HR=1.21，95%CI 0.79~1.86）；根据预定的无效边界，341 例患者入组后，研究提前关闭。
 
两组的次要疗效终点相似：ORR为16%（CR/PR，32 例 ）vs. 13%（13 例）；中位DOR为8.1个月 vs. 6.7个月；Binimetinib组和PCC组的中位OS分别为25.3个月和20.8个月。安全性结果与已知的Binimetinib安全性一致；最常见的≥ 3级不良事件是血肌酸激酶水平升高（26%）。析因分析表明，KRAS突变与对Binimetinib的反应之间可能存在关联。
