研究认为动脉瘤性蛛网膜下腔出血后谷氨酰胺水平降低导致医院获得性感染风险增加

近日研究人员就动脉瘤性蛛网膜下腔出血(SAH)后血清谷氨酰胺水平变化及其与医院获得性感染(HAIS)之间的关系进行了考察。

SAH患者在出血后14天内测定每日热量摄入量和C反应蛋白、转胸腺肽、肿瘤坏死因子受体1a(TNFαr1a)、谷氨酰胺和氮平衡水平。通过多变量模型考察血清谷氨酰胺水平与 HAI之间的关系，每日追踪HAIS并进行时间-事件分析，SAH后3个月评估患者认知和Rankin得分。

77名患者参与研究，平均年龄55岁，SAH后8 ± 3天，发生18例(23%)HAI。多因素线性评估显示，负氮平衡以及TNFαR1a升高导致血清谷氨酰胺水平增加。14天血清谷氨酰胺水平降低人群(166 ± 110 vs 236 ± 81 μg/mL， p = 0.004)HAI风险增加。入院Hunt-Hess评分以及血清谷氨酰胺水平低增加HAI风险。14天平均血清谷氨酰胺水平较低与认知得分下降以及3个月Rankin评估差相关。

研究认为动脉瘤性蛛网膜下腔出血后谷氨酰胺水平降低导致医院获得性感染风险增加。
