European Radiology:人工智能,实现COVID-19的“一站式”诊断及检测

现阶段,胸部CT扫描已成为诊断COVID-19的一项重要手段,但由于COVID-19的某些CT特征与其他病毒性感染相似,因此诊断准确性有待商榷。

目前,使用聚合酶链式反应(PCR)的COVID-19诊断金标准仍有一定的局限性,其准确性也取决于患者的临床状况、取样技术和使用的检测试剂盒。现阶段,胸部CT扫描已成为诊断COVID-19的一项重要手段,但由于COVID-19的某些CT特征与其他病毒性感染相似,因此诊断准确性有待商榷。

为了克服这些挑战,发表在European Radiology杂志的一项研究开发了一个用于COVID-19CT分析的深度学习(DL)系统,称为CoviDet,该系统包括一个自动分割算法,以测试其对连续CT图像中对COVID-19变化进行自动监测和预测的能力,这将为加快针对COVID-19的全球合作和完善模型的普适性带来机会。

本研究采用来自6个不同的多中心队列的CT成像进行逐步诊断算法,以诊断COVID-19。将有3个以上连续CT图像的患者用于算法监测。FL应用于独立构建的DL模型的分散细化。 

本研究共使用了4804名患者的1552988张CT片。该模型可根据CT单独诊断COVID-19,AUC为0.98(95%CI为0.97-0.99),并优于放射科医生的评估。本研究还成功测试了DL诊断模型与FL框架的结合。其自动分割分析与放射科医生的分析有很好的相关性,并达到了0.77的高Dice系数。如果有连续的CT评估,该系统可以产生对病人临床过程的预测曲线。

本研究表明,无论是否有临床数据根据,CoviDet均可以根据CT扫描准确诊断出COVID-19,因此本结果提示可进一步将CoviDet用于监测和预测患者的临床疾病进程及预后。
