Sci Rep:鼻腔微生物组多样性和炎症模式对鼻息肉预后的影响

最近,有研究人员为了解慢性鼻窦炎伴息肉(CRSwNP)的炎症微环境和微生物组因素对预后的影响探讨了复发组和非复发组鼻窦微生物组和炎症细胞因子的特点差异

研究人员共收集了77名CRSwNP患者的鼻腔分泌物和息肉组织。然后,他们从棉签中提取微生物DNA,进行基于16S rRNA的高通量测序来检测细菌群落组成,并使用Luminex分析息肉组织中的IL-5、IL-8、IL-17a、IL-17e、IL-18、IL-27和INF-gamma等细胞因子。研究人员还对外周血和息肉组织中的嗜酸性细胞和中性粒细胞进行计数;对CRSwNP患者进行术后随访1年来记录鼻息肉复发情况,并分析鼻息肉复发情况与炎症细胞因子、炎症细胞计数、鼻腔微生物多样性等特征的相关性。

结果表明,1年的随访中共有复发患者12名,其中男性5例,女性7例。CRSwNP患者术后鼻息肉复发与年龄、性别、哮喘、过敏性鼻炎或其他过敏性疾病无明显相关性。在鼻部症状总评分方面,复发组明显高于非复发组。在鼻息肉组织中,复发组CRSwNP患者的嗜酸性粒细胞(40.83/HP)和中性粒细胞(30.83/HP)数明显高于非复发组(13.72/HP);复发组中性粒细胞(18.5/HP)也明显高于非复发组。复发组IFN-、IL-17A、IL-17E、IL-18的表达水平明显高于非复发组,阳性率无差异。

另外,在中国西南地区,肠杆菌和厌氧菌可能与鼻息肉的炎症模式表达相关中性粒细胞介导的炎症反应在中国西南地区CRSwNP患者中起着重要作用,并与鼻息肉复发相关鼻内镜鼻窦手术后鼻息肉的复发可能与保护性微生物的丰度降低和致病性微生物的数量增加有潜在关系。
