JNER:虚拟现实、机器人和电刺激联合干预上肢卒中康复的可行性和初步疗效

结果表明虚拟现实、机器人和电刺激有助于改善中风所致中重度损害患者的UE功能。

大约80%的中风患者经历了上肢偏瘫(UE),导致慢性损伤,如失去运动控制、疼痛和痉挛。这些对生活质量有重要影响,因为手和手臂功能的损害限制了日常生活活动的参与。因此,慢性中风患者、护理人员和康复专业人员认为UE功能的恢复非常重要。UE康复应让患肢参与“有意义、参与、重复、逐步适应、特定任务和目标导向的训练”。康复技术的进步,特别是机器人技术、虚拟现实技术(VR)和神经肌肉电刺激技术(NMES),已被证明通过提供此类重复和任务导向的训练,对改善中风患者的UE功能是有效的。

中风参与者(n = 28)接受为期4周的干预(每周三次训练),皮质脊髓束的功能完整性适用生理电信号进行评估,包括运动诱发电位测量和手动肌肉测试。手部恢复潜力低的患者(肩关节组;n = 18) 接受了只关注肘部和肩部运动的机器人康复干预。具有良好手部康复潜力的患者(手组;n = 10) 除了机器人治疗外,还接受了肌电图触发的NME治疗(电刺激)。总之,肩部组的参与者接受了60分钟的机器人治疗。一些能够克服重力移动手臂的人也接受了虚拟现实干预(例如,30分钟的机器人治疗和30分钟的虚拟现实治疗)。手部组的参与者接受了大约30分钟的机器人治疗,然后是30分钟的VR治疗和NMES。主要结果为Fugl Meyer UE评估和Abilihand评估。次要结果包括运动活动日志和中风影响量表。

机器人康复系统的总体视图

在本研究中,确定了个性康复计划干预的可行性,并测量了结合机器人技术、VR和NMES改善慢性卒中患者UE功能的初步疗效。表明,康复效果是安全的,并且受到参与者的欢迎,患者认为机器人技术和虚拟现实活动都是令人愉快的,并且是有益的。在康复训练后,18名参与者(80%的手部组(10名中的8名)和56%的肩部组(18名中的10名))的主要结果(FMA-UE或ABLIHAND)有所改善。大多数在与UE活动性能相关的其他措施(SIS或MAL-14)中也有所改善。 然而,很少或没有参与者在手部力量或灵巧度(BBT、握力、SIS手)方面有改善。虽然肩关节组的参与者没有进行任何与手相关的练习,但康复项目似乎对手关节组的参与者的手功能影响有限。这可能是由于试验次数不足或手部任务缺乏可变性所致。
