动脉粥样硬化和骨质疏松症是与年龄相关的两大疾病,主要发生于中老年女性,导致其死亡率明显升高。动脉粥样硬化包括导致动脉粥样硬化性血管疾病(ASCVD)的多因素过程,这是全球的主要死亡原因之一。骨质疏松症的特征是骨矿物质密度(BMD)降低和骨骼结构恶化,使骨折风险增加。

本研究旨在评估骨矿物质密度(BMD)是否可为预测女性发生动脉粥样硬化性心血管疾病的风险提供独立的预测价值

共回顾性地分析了12681位50-80岁(平均63岁)的接受了双重X线骨密度仪检查的女性。根据BMD或临床骨质疏松症诊断评估ASCVD事件(ASCVD死亡、非致死刑心肌梗死和缺血性卒中)的风险比(HR),并根据临床风险因素(包括年龄、体重指数、高血压、2型糖尿病、高脂血症、吸烟和骨折史)进行校正。此外,研究人员还评估了BMD或骨质疏松症诊断加临床风险因素是否可提高对ASCVD事件的预测。

不同部位BMD降低对ASCVD事件风险的影响

总体上,在随访期间(中位 9.2年),共468位女性(3.7%)发生了ASCVD事件。腰椎、股骨颈和全髋关节的BMD降低与ASCVD事件的高风险独立相关(BMD每降低1个标准差的校正HR分别是:1.16, p<0.001; 1.29, p<0.001; 1.38, p<0.001)。

确诊骨质疏松症也与ASCVD事件的较高风险独立相关(校正HR:1.79,p<0.001)。临床风险因素联合BMD或骨质疏松症在鉴别ASCVD事件方面具有显著更高的价值。
